MasonⅡ型桡骨小头骨折三种内固定的生物力学特性

【摘要】 [目的]探讨三种内固定方式治疗MasonⅡ型桡骨小头骨折的生物力学特征,寻找最佳内固定方法,为临床应用提供基础理论依据。[方法]运用3D打印技术制作20具桡骨标本,随机分为四组:正常对照组5个标本、克氏针组5个、Bold螺钉组5个、微型钢板组5个,并在后三组的15个标本建立MasonⅡ型桡骨小头骨折模型,给予相应固定。测定比较三种内固定方法治疗桡骨小头骨折的生物力学指标。[结果]三个骨折-固定组的轴向压缩强度(σc)分别与正常对照组比较下降23%、7%和2%;三组的轴向压缩刚度(EF)分别与正常对照组比较降低26%、10%和4%; Bold螺钉组、微型钢板组分别与正常对照组比较差异无统计学意义,而克氏针组与正常对照组两组比较差异有统计学意义,微型钢板组生物力学性能最佳,Bold螺钉组其次,克氏针组最差。[结论]MasonⅡ型桡骨小头骨折采用微型钢板固定具有优越的生物力学性能,比其他两组固定效果更好。相比之下Bold螺钉固定优于克氏针固定。

【Abstract】 [Objective] To explore the biomechanical characteristics of three internal fixation methods for the treatment of Mason type II fracture of the radial head, and to find the best internal fixation method to provide the theoretical basis for clinical application. [Methods] Twenty radius specimens were made by using 3 D printing technology and randomly divided into 4 groups, including 5 in the normal control group, 5 in Kirschner wire group, 5 in the Bold screw group and 5 in mini plate group.Of them, 15 specimens in the latter 3 groups were created Mason type II radial head fracture model, and were fixed by corresponding methods. Biomechanical parameters measured in these specimens were compared among the 4 groups. [Results] The axial compressive strength(σc) in the 3 fracture-fixation groups decreased by 23%, 7% and 2% respectively compared with the normal control group. The axial compressive stiffness(EF) of the three groups reduced by 26%, 10% and 4% respectively compared with the normal control group. There was no significant difference between Bold screw group and miniature steel plate group and normal control group, but there was significant difference between Kirschner wire group and normal control group.The biomechanical properties of miniature steel plate group were the best, followed by Bold screw group and Kirschner wire group. [Conclusion] The mini plate fixation has superior biomechanical properties to other two fixation methods for Mason type II fracture of the radial head. In addition, the Bold screw fixation is superior to the Kirschner wire.
