红外线计算机辅助导航的全膝关节置换术

【摘要】 [目的]探讨应用红外线计算机导航下行全膝关节置换术(TKA)治疗终末期骨关节炎的临床效果。[方法]筛选2017年1月~2018年1月于苏州市立医院关节外科住院因终末期膝关节骨性关节炎行初次单侧TKA的患者46例为研究对象,将其随机分为导航组和常规组。导航组患者采用红外线计算机导航下的TKA,常规组患者采用传统TKA。对比两组患者手术情况及术后并发症、软组织平衡、功能评分及术后影像资料。[结果]导航组的手术时间长于常规组,其术中出血量、术后24 h引流量均明显低于常规组,差异有统计学意义(P<0.05)。末次随访时,导航组术后膝关节活动度大于常规组(P<0.05),但两组的AKSS及Oxford评分对比差异无统计学意义(P>0.05)。导航组术后下肢力线误差、软组织平衡角度变量及软组织平衡分离变量均小于常规组(P<0.05);导航组患者置入假体的α、β、γ、δ角度偏差显著低于常规组患者(P<0.05)。[结论]红外线计算机导航辅助下TKA安装假体的位置角度更为精准。

【Abstract】 [Objective] To investigate the application of infrared computer-assisted navigation in total knee arthroplasty(TKA). [Methods] A total of 46 patients who were candidate of first unilateral total knee arthroplasty in our department from January of 2017 to January of 2018 were included into this study, and divided randomly into two groups equally. Of them, 23 patients had TKA performed under infrared computer-assisted navigation, while the remaining 23 patients received conventional TKA. The perioperative data and postoperative complications, as well as range of motion(ROM), AKSS score, Oxford score and radiographic measurements at follow-ups were compared between the two groups. [Results] The navigation group spend significantly longer operation time, whereas had significantly less intraoperative blood loss and postoperative 24-hour drainage volume than the conventional group(P<0.05). At the latest follow-up, the navigation group achieved significantly greater ROM than the conventional group(P<0.05), although no statistically significant differences regarding to AKSS and Oxford scores were proved between them(P>0.05). In term of radiographic measurements, the navigation group had significantly less alignment deviation, soft tissue balance angle deviation and distance deviation than the conventional group(P<0.05). In addition,the navigation group got significantly less prosthetic placement deviations, such as α、β、γ、δ angles, than the conventional group(P<0.05). [Conclusion] Under infrared computer-assisted navigation, the TKA prostheses do be placed in more accurate position.
