不同水溶性维生素对新生儿肠外营养液稳定性和临床疗效的影响
The effects on the stability and clinical efficacy of neonatal parenteral nutrition containing different water-soluble vitamins
【摘要】 目的:比较含不同厂家水溶性维生素的新生儿肠外营养液的稳定性,并评价其在临床使用的疗效。方法:采用实验室检测肠外营养液的外观性状、pH、乳滴平均粒径及分布、不溶性微粒数和细菌生长情况的方法考察其稳定性;采用回顾性分析肠外营养治疗前后的体质量、住院时间、日均体质量增长率、生化指标值、并发症发生率考察临床疗效。结果:含不同水溶性维生素的肠外营养液的pH、不溶性微粒数、乳滴平均粒径的差异无显著性(P>0.05);PDI值的差异具有显著性(P<0.05),不含对羟基苯甲酸甲酯的肠外营养液的乳滴单分散性相对较好。使用含不同水溶性维生素肠外营养液的患儿在肠外营养支持前后的体质量、营养学指标、肝功能指标、血糖(Glu)以及电解质含量的差异无显著性(P>0.05);使用含相同水溶性维生素肠外营养液的患儿在肠外营养支持前后的体质量、白蛋白(ALB)、前蛋白(PA)以及总蛋白(TP)、血糖(Glu)以及非结合胆红素(IB)的差异均有显著性(P<0.05)。结论:不同厂家水溶性维生素均可用于新生儿肠外营养液,均有利于改善患儿的营养状况,但注射剂中抑菌剂的安全性应引起重视。
【Abstract】 OBJECTIVE To compare the stability of water-soluble vitamins in neonatal parenteral nutrition solutions from different manufacturers and to evaluate their clinical efficacy. METHODS The appearance, pH average size and distribution of emulsion droplets, insoluble particle number and bacterial growth of parenteral nutrition solution were detected in the laboratory.. A retrospective analysis was conducted to examine clinical efficacy by body mass, hospitalization time, daily body mass growth rate, biochemical index value, and complication rate before and after parenteral nutrition treatment. RESULTS There were no significant differences in pH value, insoluble particle number and average droplet size of nutrition containing different water-soluble vitamins(P>0.05), and the difference of PDI value was statistically significant(P<0.05), the distribution of nutrition without methylparaben was relatively well. There was no significant difference in body weight, nutritional indicators, liver function indicators, blood glucose(Glu) and electrolyte content between the children before and after parenteral nutrition support(P>0.05) who use parenteral nutrition containing different water-soluble vitamins, there were significant differences in body weight, albumin(ALB), proprotein(PA), total protein(TP), blood glucose(Glu), and unconjugated bilirubin(IB) between the children before and after parenteral nutrition support(P<0.05) who used parenteral nutrition containing same water-soluble vitamins. CONCLUSION Water-soluble vitamins of different manufacturers can be used in neonatal parenteral nutrition, which are beneficial to improve the nutritional status of neonate. However, attention should be paid to the safety of bacteriostatic agents in injections.
