急性心肌梗死预后评估指标的研究进展

【摘要】 可溶性生长刺激表达基因2蛋白(s ST2)和氨基末端脑钠肽前体(NT-proBNP)都可作为急性心肌梗死(AMI)患者预后的独立预测因子,可以预测AMI患者的主要不良心血管事件,包括心肌梗死再入院、梗死后心力衰竭、心血管死亡等。但是NT-proBNP易受临床因素如年龄、性别及心肌梗死史等的影响,而s ST2能独立于这些临床因素。研究显示,多生物标志物联合检测的预后价值往往比单生物标志物检测的预后价值要高,sST2和NTpro BNP之间有相互补充的作用,两者联合检测对AMI患者预后评估更具优越性。本文对s ST2、NT-proBNP以及两者联合检测对AMI患者预后的临床价值作一综述。

【Abstract】 Both soluble growth stimulation expressed gene 2(sST2) and N-terminal pro-B-type natriuretic peptide(NT-proBNP) can be used as independent predictors of prognosis in patients with acute myocardial infarction(AMI),and can predict major adverse cardiovascular events in patients with AMI, including myocardial infarction readmission,post-infarction heart failure, cardiovascular death, etc. However, NT-proBNP is susceptible to clinical factors such as age, gender, and history of myocardial infarction, and sST2 can be independent of these clinical factors. Studies have shown that the prognostic value of combined detection of multiple biomarkers is often higher than the prognostic value of single biomarker detection, and there is a complementary effect between sST2 and NT-proBNP. The combined detection of sST2 and NT-proBNP is superior to the prognosis of patients with AMI. This article reviews the clinical value of sST2, NT-proBNP and their combined detection in the prognosis of patients with AMI.
