白细胞介素-1β、白细胞介素-8及肿瘤坏死因子-α与房颤伴凝血功能异常的相关性

【摘要】 目的研究白细胞介素-1β(IL-1β)、白细胞介素8(IL-8)及肿瘤坏死因子-α(TNF-α)与心房颤动(房颤)伴凝血功能异常的关系。方法选取2016年1月~2017年12月我院收治的42例房颤患者作为房颤组,其中阵发性房颤14例,持续性房颤28例,另选取同期门诊体检的20例健康者作为对照组。采用酶联免疫法检测各组的血浆IL-1β、IL-8、TNF-α水平,分析其与房颤类型、凝血功能参数及血常规参数的相关性。结果房颤组与对照组的IL-1β水平比较,差异无统计学意义(P>0.05);房颤组的IL-8、TNF-α水平均明显高于对照组,差异有统计学意义(P<0.05)。阵发性房颤组、持续性房颤组、对照组的IL-1β水平比较,差异无统计学意义(P>0.05);阵发性房颤组、持续性房颤组的IL-8及TNF-α水平均明显高于对照组,差异有统计学意义(P<0.05)。不同凝血酶原时间、国际标准化比值情况下的IL-1β、IL-8、TNF-α水平比较,差异无统计学意义(P>0.05);凝血酶时间>16 s患者的IL-1β、TNF-α水平均明显高于凝血酶时间≤16 s(正常)患者,差异有统计学意义(P<0.05)。IL-1β、TNF-α水平与白细胞计数、红细胞计数、血小板计数、中性粒细胞百分比、淋巴细胞百分比、单核细胞百分比均无相关性(P>0.05);IL-8水平与淋巴细胞百分比成正相关(r=0.314,P<0.05)。结论 IL-8及TNF-α水平在房颤患者中增高,提示其可能与房颤发生相关;IL-1β、TNF-α水平增高与房颤伴凝血功能异常相关。

【Abstract】 Objective To study the relationship between interleukin-1β(IL-1β), interleukin-8(IL-8), tumor necrosis factor-α(TNF-α) and atrial fibrillation with abnormal coagulation function. Methods A total of 42 patients with atrial fibrillation admitted to our hospital from January 2016 to December 2017 were selected as the atrial fibrillation group,including 14 patients with paroxysmal atrial fibrillation, 28 patients with persistent atrial fibrillation, and 20 healthy subjects who underwent physical examination at the same time were selected as the control group. The levels of IL-1β,IL-8 and TNF-α in plasma were detected by enzyme linked immunosorbent assay, and their correlation with atrial fibrillation type, coagulation function parameters and blood routine parameters were analyzed. Results There was no significant difference in IL-1β level between the atrial fibrillation group and the control group(P>0.05). The levels of IL-8 and TNF-α in the atrial fibrillation group were significantly higher than those in the control group, and the differences were statistically significant(P<0.05). There was no significant difference in IL-1β level between paroxysmal atrial fibrillation group, persistent atrial fibrillation group and control group(P>0.05). The levels of IL-8 and TNF-α in paroxysmal atrial fibrillation group and persistent atrial fibrillation group were significantly higher than those in the control group, and the differences were statistically significant(P<0.05). There were no significant differences in the levels of IL-1β, IL-8 and TNF-α between different prothrombin time and international normalized ratio(P >0.05). The levels of IL-1β and TNF-α in patients with thrombin time >16 s were significantly higher than those with thrombin time ≤16 s(normal), and the differences were statistically significant(P<0.05). There was no correlation between IL-1β, TNF-α levels and white blood cell count, red blood cell count, platelet count, neutrophil percentage, lymphocyte percentage, and monocyte percentage(P>0.05). The IL-8 level was positively correlated with lymphocyte percentage(r=0.314, P<0.05). Conclusion The elevated levels of IL-8 and TNF-α in patients with atrial fibrillation suggest that they may be related to the occurrence of atrial fibrillation,and the elevated levels of IL-1β and TNF-α are related to the abnormal coagulation function in patients with atrial fibrillation.
