钠-葡萄糖协同转运蛋白2抑制剂的临床应用研究进展
Review of the application of sodium-glucose co-transporter type 2 inhibitor
【摘要】 钠-葡萄糖协同转运蛋白2(SGLT-2)抑制剂在国内已上市,其降糖机制独特,同时还具有减少心脑血管事件,减轻体重、IR,减少体脂,降低血压、尿酸,保护肾功能、胰岛β细胞等作用。该药运用至今安全性良好,单独运用时低血糖风险小。泌尿和生殖系统感染是其主要不良反应。虽然可能增加DKA、骨折和骨质疏松的风险,也有急性肾功能损伤和膀胱癌的报道,但因其独特的降糖机制和被证实的临床获益,只要选择适宜人群并充分掌握其特点,在未来的糖尿病个体化治疗中必能发挥良好疗效。
【Abstract】 Sodium-glucose co-transporter type 2(SGLT-2)inhibitors have been marketed in China.With their unique mechanism,they are also beneficial to reduce cardiovascular and cerebrovascular events,lose weight and body fat,lower blood pressure and uric acid,maintain kidney function and improve insulin resistance and β-cell function. The drug has been used safely so far,and the risk of hypoglycemia is lower when monotherapy is used. Urinary and genital infections are the major side effects,although there is a potential for increased risk of DKA,fractures and osteoporosis,as well as acute renal impairment and bladder cancer. Due to its unique hypoglycemic mechanism and proven clinical benefit,if we can choose the right patients and fully understand the characteristics of SGLT-2 inhibitors,it will be able to play a good role in the future individualized treatment of diabetes.
