枸橼酸咖啡因与氨茶碱在早产儿原发性呼吸暂停中的临床应用对比
Clinical application comparison of caffeine citrate and aminophylline in primary apnea of premature infants
【摘要】 目的探讨枸橼酸咖啡因在早产儿原发性呼吸暂停中的临床观察。方法选取2014年1月至2018年6月出生的原发性呼吸暂停早产儿98例进行前瞻性研究,随机数表法分成对照组和观察组,每组49例。在常规处理基础上对照组予以氨茶碱药物治疗,而对观察组则予以枸橼酸咖啡因药物治疗。干预治疗3 d后,分别对两组患儿的呼吸暂停发生次数、治疗总有效率以及不良反应的发生情况进行比较。结果观察组患儿治疗1 d、2 d、3 d后的呼吸暂停次数均显著少于对照组患儿,差异有高度统计学意义(t=-5.796、-4.745、-14.033,P<0.01);观察组患儿的治疗总有效率为83.7%(41/49),明显高于对照组患儿的55.1%(27/49),差异有统计学意义(χ2=5.167,P=0.023);且观察组患儿的不良反应发生率10.2%(5/49)显著低于对照组患儿的42.9%(21/49),差异有统计学意义(χ2=8.642,P=0.003)。结论采用枸橼酸咖啡因对早产儿原发性呼吸暂停进行治疗,具有起效快且安全性高的特点,治疗效果显著优于氨茶碱。
【Abstract】 Objective To observe the clinical effect of caffeine citrate in primary apnea of premature infants.Methods98 cases of primary apnea premature infants born from January 2014 to June 2018 were randomly divided into the control group and the treatment group,with 49 cases in each group.Besides routine treatment,infants in the control group were treated with aminophylline while patients in the treatment group were treated with caffeine citrate.After 3 days of intervention,the frequency of apnea,total effective rate and adverse reactions were compared between the two groups.Re s ults 1 d,2 ds and 3 ds after the intervention treatment,the apnea times of the treatment group were significantly fewer than those of the control group and the difference was statistically significant( t =-5.796,-4.745,-14.033,P<0.01).The total effective rate of the treatment group[83.7%,(41/49)] was significantly higher than that of the control group and the difference was of statistical significance [55.1%,(27/49)](χ2 = 5.167,P = 0.023).The adverse reactions incidence rate in the treatment group was significantly lower than that of the control group [10.2%,(5/49)vs.42.9%,(21/49)] and the difference was statistically significant( χ2 = 8.642,P = 0.003).Conclus ion The use of caffeine citrate in the treatment of primary apnea in premature infants had the characteristics of being quickly effective and safe and the treatment effect was significantly better than that of aminophylline.
