二维斑点追踪技术评价2型糖尿病合并非酒精性脂肪性肝病患者左心房功能
Two-dimensional Speckle Tracking Echocardiography in Evaluating the Left Atrial Function of the Type 2 Diabetes Mellitus Patients with Nonacoholic Fatty Liver Disease
【摘要】 目的应用二维斑点追踪技术(2D-STE)评价2型糖尿病(T2DM)合并非酒精性脂肪性肝病(NAFLD)患者的左心房功能。方法收集T2DM患者97例,依据其肝脏超声表现分为3组,A组(对照组)30例,未合并NAFLD,B组32例,合并轻度NAFLD,C组35例,合并中-重度NAFLD。常规超声测量左心房收缩末期前后径(LAD)、左心房收缩期末最大容积、舒张期末最小容积(LAVmax、LAVmin)、左心房收缩前容积(LAVp)并得出左心房主动、被动射血分数(LAAEF、LAPEF)及最大容积指数(LAVImax)。二维斑点追踪技术(2D-STE)测量左心房收缩期、舒张早期及晚期整体纵向峰值应变率(LASRs、LASRe、LASRa)。结果与A、B组相比,C组LAAEF和LASRa明显增高(均P<0.05),LAPEF、LASRe、LASRs明显降低(均P<0.05)。B组各参数与A组相比,差异无统计学意义(均P>0.05)。多元线性回归分析显示,NAFLD是左心房功能改变的独立危险因素。结论 2D-STE可以评价T2DM合并NAFLD患者的左心房功能。
【Abstract】 Objective To assess left atrial function in type 2 diabetes mellitus(T2DM)patients with nonalcoholic fatty liver disease(NAFLD)by two-dimensional speckle tracking echocardiography(2D-STE).Methods 97 type 2 diabetes mellitus(T2DM)patients totally were divided into GroupA(without NAFLD in conrol group),GroupB(mild fatty liver)and GroupC(moderate to severe fatty liver),according to the diagnostic criteria of non-alcoholic fatty liver disease.Conventional echocardiography parameters included left atrial end-systolic diameter(LAD),LV ejection fraction(LVEF)and LAvolumes(max,min and pre-atrial contraction volume),then calculated LA passive and active ejection fraction(LAPEF,LAAEF),left atrial maximum volume index(LAVImax).The global peak longitudinal systolic strain(LASRs),early diastolic strain(LASRe)and late diastolic strain(LASRa)rates of the LA were obtained by 2D-STE.Results There was no significant difference between group A and group B(all P> 0.05).In group C,LAAEF and LASRa were higher,while LAPEF,LASRe and LASRs were lower compared with the corresponding values in groups A and B(all P<0.05).By the results of multivariate linear regression analysis:NAFLD was an independent risk factor for the left atrium function changes.Conclusions 2D-STE can evaluate the left atrial function in T2DM patients with NAFLD.
