【摘要】 目的探讨超声引导下¹²⁵I粒子植入治疗甲状腺癌术后淋巴结转移癌的疗效。方法选取我院收治的38例甲状腺癌术后淋巴结转移患者（共69个转移淋巴结）作为研究对象,在超声引导下¹²⁵I粒子植入治疗。分别于1、3、6个月及12个月观察患者治疗前后甲状腺球蛋白（TG）水平、淋巴结最大径变化及淋巴结内部血流变化。结果 TG水平在治疗后的1个月中下降尤为明显,中位水平由治疗前的25.14μg/L降为7.97μg/L （P<0.05）,6～12个月下降缓慢,中位水平由0.65μg/L降至0.45μg/L （P<0.05）。3组患者在不同时间点淋巴结最大径变化有统计学意义（P<0.05）,淋巴结最大径在治疗后1个月中变化不明显（P=0.13、0.21、0.09）,3组组间两两对比中差异均无统计学意义,缓解率为81.6%,局部控制率为100%。淋巴结治疗后1个月内部血流明显降级（Z=11.82,P<0.05）,患者治疗后均未出现严重并发症及不良反应。结论超声引导¹²⁵I粒子植入治疗甲状腺癌术后淋巴结转移疗效显著,为甲状腺癌术后淋巴结转移不能再次手术及¹³¹I治疗的患者提供了一项有效的微创治疗手段。更多还原

【Abstract】 Objective The efficacy and safety of ultrasound guided ¹²⁵I implantation in the treatment of lymph node metastasis after thyroid carcinoma were discussed.Methods 38 patients（A total of 69 metastatic lymph nodes）who underwent postoperative thyroid carcinoma with lymph node metastasis admitted from August 2015 to August 2016 in our hospital were enrolled in this study and were treated with ¹²⁵I particles implantation under ultrasound guidance.The levels of TG,the maximum diameter of lymph nodes and the changes of blood flow inside lymph nodes were observed before and after treatment at 1,3,6 and 12 months,respectively.Results The level of TG decreased significantly in the 1 months after treatment,and the median level decreased from 25.14μg/L before treatment to 7.97μg/L（P<0.05）.It decreased slowly from 6 month to 12 month,and the median level decreased from 0.65μg/L to 0.45μg/L（P<0.05）.The maximum diameter of lymph nodes did not change significantly in 1 months after treatment（P=0.13,0.21,0.09）.There was no significant difference between the three groups.The remission rate was 81.6%,and the local control rate was 100%.Conclusions Ultrasound-guided ¹²⁵I particle implantation for the treatment of postoperative lymph node metastasis of thyroid cancer has a significant short-term effect,providing a safe and effective minimally invasive treatment for patients with thyroid cancer who can not undergo re-operation and ¹³¹I treatment of postoperative lymph node metastasis.更多还原
