超声引导经皮粗针切割与细针抽吸肺活检的比较

【摘要】 目的比较超声引导下粗针切割(CNB)与细针抽吸(FNA)两种方法的对肺周围疾病的诊断效果。方法回顾性分析我院96例行超声引导经皮肺活检病例。获取患者的穿刺病理结果,术后并发症及自穿刺活检到病理诊断的时间。比较两种穿刺方法在不同类型疾病的正确率、并发症发生率及获得诊断所需时间的差异。结果 96例患者中,单独使用CNB、FNA及同时使用CNB和FNA例数分别为39、8、49例,并发症例数分别为2、0、2例。CNB平均直径(5.64±2.64)cm,FNA平均直径(5.02±2.91)cm (P<0.05)。获得病理诊断所需时间分别为(3.63±2.83)d及(2.03±1.08)d (P<0.05)。CNB与FNA对恶性病变正确率分别为95.6%(44/46)及76.0%(19/25)(P<0.05),良性病变分别为93.0%(40/43)及45.7%(16/35)(P<0.05)。三种穿刺方式的并发症发生率无显著区别(P>0.05)。结论 CNB对恶性病灶及良性病灶的正确率均高于FNA,但是FNA获得诊断时间较短。三种穿刺并发症发生率没有明显的区别。

【Abstract】 Objective The purpose of this study was to compare the diagnostic effects of core needle biopsy(CNB)and fine needle aspiration(FNA)in peripheral lung diseases.Methods A retrospective analysis was conducted of 96 cases of lung biopsy with confirmed final diagnosis.The pathological results,time for pathological diagnosis,and postoperative complications were recorded.Results Of the 96 patients,39 were treated with CNB alone,8 with FNA alone,and 49 with both methods.The average times from biopsy to obtaining pathological results were(3.63±2.83)and(2.03±1.08)days in CNB and FNA,respectively(P<0.05).The numbers of malignant tumors detected by CNB and FNA were 46 and 25,respectively,with the corresponding diagnostic accuracies being 95.6%and 76%,respectively(P<0.05).The numbers of benign tumors detected by CNB and FNA were 43 and 35,with the diagnostic accuracies for benign disease being 93% and 45.7%,respectively(P<0.05).There was no significant difference among CNB,FNA,and CNB+FNA in the number of complications(2,0,and 2,respectively;P>0.05).Conclusions The diagnostic accuracy of CNB for both malignant and benign lesions was higher than that of FNA.However,FNA allowed faster diagnosis than CNB.There was no significant difference in the incidence of complications among CNB,FNA,and CNB+FNA.
