【摘要】 目的探究尿毒症患者腹膜透析合并腹膜炎感染状况及其超敏C反应蛋白(hypersensitive C-reactive protein,CRP)、降钙素原(procalcitonin,PCT)水平。方法回顾性分析2017年3月—2018年7月在上海市第一人民医院宝山分院进行腹膜透析的60例尿毒症患者的诊疗情况,根据其合并腹膜炎感染与否,分为对照组和观察组(合并腹膜炎)。对照组32例,观察组28例。检测分析两组患者PCT、CRP及白细胞计数(white blood cell,WBC)随治疗时间的变化。之后对观察组进行抗感染治疗,观察PCT、CRP水平变化及其与腹膜炎感染的相关性。结果观察组CRP、PCT水平明显的高于对照组,两组比较有统计学意义(P<0.05)。观察组治疗之后CRP、PCT水平具有显著的组间差异(P<0.05),且随着治疗时间的延长,患者CRP、PCT水平显著降低(P<0.05)。而两组WBC水平比较无明显差别,且其最高数值均未超出正常范围。合并腹膜炎感染患者PCT、CRP及WBC水平的相关性分析表明,PCT与CRP呈现正相关的关系(r=0.625,P<0.05),而PCT与WBC水平不相关。结论 CRP、PCT水平可以作为尿毒症患者腹膜透析合并腹膜炎感染状况的检测指标。更多还原

【Abstract】 OBjECTIVE To investigate the status of peritoneal dialysis with peritonitis and its PCT and CRP levels in patients with uremia.METHODS A retrospective analysis of 60 patients with uremia who underwent peritoneal dialysis in our hospital from March 2017 to July2018 was divided into control group and observation group(combined with peritonitis)according to whether they were infected with peritonitis. There were 32 cases in the control group and 28 cases in the observation group. Hypersensitive C-reactive protein(hs-CRP),procalcitonin(PCT)and white blood cell(WBC)were detected in the two groups. The observation group was then treated with anti-infective treatment,and the changes in PCT and CRP levels were observed and analyzed for correlation with peritonitis infection. RESULTS The levels of CRP,PCT and WBC in the observation group were significantly higher than those in the control group(P<0.05). The two groups were statistically significant. There was a significant difference between the groups after the observation group(P<0.05)and the CRP and PCT levels were significantly decreased(P<0.05). Correlation analysis between serum PCT,CRP and WBC levels in patients with peritonitis infection showed that there was a significant positive correlation between PCT and CRP(r=0.625,P<0.05),and PCT had no correlation with WBC. CONCLUSION The levels of CRP and PCT could be used as indicators for the detection of peritoneal dialysis with peritonitis in patients with uremia.更多还原
