双源CT定量成像评估肺癌患者术后肺功能改变的应用效果

【摘要】 目的探讨双源CT定量成像技术对肺癌患者术后肺功能改变的临床应用价值。方法选取2016年3月~2018年3月行单操作孔胸腔镜下肺叶或全肺切除治疗的肺癌患者30例为观察对象。分别于术前7 d以及术后3个月采用Vmax Series V6200 Autobox型肺功能仪器检测所有患者的肺功能情况。此外,对所有患者予以双源CT定量成像技术扫描,分析患者CT容积扫描、肺密度指标和肺功能的相关性。同时,分析不同肺叶吸气相与术后肺功能的相关性。结果通过Pearson相关性分析发现:Vin、Vex、Vin-Vex、MLDin(HU)、MLDex(HU)、MLDin-MLDex(HU)与手术前后的TLC、RV、FVC、FEV1、FEV1/FVC均存在明显相关性。通过Pearson相关性分析发现:右上叶、右中叶、右下叶、右全肺、左上叶、左下叶、左全肺吸气相与术后TLC、FVC、FEV1水平均存在显著相关性,且右上叶、右下叶、左上叶吸气相与术后FEV1/FVC水平存在显著相关性。结论双源CT定量成像技术可用于肺癌患者术后肺功能的检测,可为医护人员提供精确的肺癌患者接受手术治疗后的肺功能变化情况。

【Abstract】 Objective To study the clinical application value of dual-source CT quantitative imaging technology in postoperative pulmonary function changes of patients with lung cancer.Methods From March 2016 to March 2018, 30 patients with lung cancer who were treated with single-operation thoracoscopic lobectomy or total lung resection in our hospital were object observation. The pulmonary function of all patients was tested by Vmax Series V6200 Autobox at 7 days before surgery and 3 months after surgery. All patients were scanned by dual-source CT quantitative imaging technology, and the correlation between CT volume scan, lung density index and lung function was analyzed.At the same time, the correlation between different lung leaf aspiration phase and postoperative lung function was analyzed. Results According to Pearson correlation analysis, there was significant correlation between Vin, Vex, vin-vex, MLDin(HU), MLDex(HU) and TLC, RV, FVC, FEV1 and FEV1/FVC before and after surgery.Pearson correlation analysis showed that there was a significant correlation between inspiration of right upper lobe, right middle lobe, right lower lobe, right lung, left upper lobe, left lower lobe, and left whole lung, and between aspiration of right upper lobe, right lower lobe and left upper lobe and FEV1 level after operation.Conclusion Dual-source CT quantitative imaging technology can be used to detect lung function in lung cancer patients after surgery, and can provide accurate changes in lung function in lung cancer patients after surgery.
