颅内动脉重度狭窄或闭塞患者认知功能损害研究
Study of cognitive impairment in patients with severe stenosis or occlusion of intracranial artery
【摘要】 目的 研究颅内动脉重度狭窄或闭塞对患者认知功能的影响,旨在早期发现患者认知功能损害及其原因,有助于临床采取有效干预措施。方法 选择2013年9月-2016年5月期间中国人民解放军火箭军总医院"脑卒中医疗救治研究中心"门诊和住院患者,共计242人。选取正常组53人,狭窄组63人,对所有患者进行血管狭窄程度评估、根据基于3T-MRI FLAIR序列的ASPECTS和pc-ASPECTS评分剔除影响关键认知功能颅内动脉供血区梗死患者,应用传统的MMSE、MoCA量表和多维心理评估系统对患者进行神经心理评估。结果 狭窄组患者MMSE、MoCA、简单数字计算、词语辨析测验、数字序列推理、空间工作记忆测验等量表评分均低于正常组患者(P <0. 05)。结论 研究表明重度颅内动脉狭窄或闭塞与患者认知功能损害相关,因此,对颅内动脉重度狭窄或闭塞患者进行早期干预和治疗具有重要意义。
【Abstract】 Objective Aim to detect the cognitive impairment and its reason of patients early,we researched the cognitive function of patients with severe stenosis or occlusion of intracranial artery,which could help the implement of effective clinical intervention. Methods All patients were chosen from out-patient or be hospitalized in"Medical treatment center for stroke",General Hospital of the PLA Rocket Force,during the period of September 2013-May 2016. There were 53 patients in the control group and 63 patients in the stenosis group. The stenosis degree of all patients was evaluated,and patients with infarction in intracranial arterial blood supply area that influenced the key cognitive function were eliminated according to the ASPECTS and pc-ASPECTS score based on 3 T-MRI FLAIR sequence. Neuropsychological assessment was performed with traditional MMSE,MoCA scale and multidimensional psychological assessment system. Results The scores of MMSE,MoCA,simple digital computing,word discrimination test,digital sequence reasoning,spatial working memory test scale score were lower than normal group( P < 0. 05). Conclusion Study showed that severe intracranial artery stenosis or occlusion are associated with cognitive impairment in patients,therefore early intervention and treatment for patients with severe intracranial artery stenosis or occlusion is of great significance.
