【摘要】 目的探讨多层螺旋CT在结肠癌诊断及术前分期中的临床价值。方法 2017年1月～2018年1月,我院对50例结肠癌患者应用多层螺旋CT检查,观察CT表现及对CT检查与术后病理诊断的符合率,CT结肠癌术前T分期及N分期与病理结果的对比。结果 CT表现为肠壁增厚,密度均匀,增强后均匀强化38例,且伴有肠腔不规则、偏心性狭窄12例;腹腔淋巴结转移11例,其中肠系膜淋巴结肿大6例,盆腔淋巴结肿大2例,腹腔淋巴结肿大3例。肝脏转移3例,肺部转移1例。术后CT检查显示乙状结肠癌15例,升结肠癌25例,降结肠癌10例,经病理证实,其中乙状结肠癌13例,升结肠癌27例,降结肠癌10例,CT诊断的符合率为92.0%(46/50)。CT结肠癌术前T分期、N分期与病理结果对比显示,采用非参数检验,差异无显著性(P>0.05)。结论多层螺旋CT用于结肠癌诊断的中可以提高诊断准确性,且对明确术前分期具有重要的参考价值。更多还原

【Abstract】 Objective To investigate and analyze the clinical value of multi-slice spiral CT in the diagnosis and preoperative staging of colon cancer. Methods From January 2017 to January 2018, 50 patients with colon cancer were given multi-slice spiral CT examination in our hospital. The CT findings and the coincidence rate between CT results and postoperative pathological diagnosis were observed. T stage and N stage of colon cancer by CT examination before the surgery of colon cancer were compared with the pathological results. Results CT findings were thickening of the intestinal wall and uniform density. After enhancement, 38 cases were uniformLy enhanced, and 12 cases were accompanied with irregular enteric cavity and eccentric stenosis; there were 11 cases of abdominal lymph node metastasis, including 6 cases of mesenteric lymphadenopathy, 2 cases of pelvic lymphadenopathy, and 3 cases of abdominal lymphadenopathy. There were 3 cases of liver metastasis and 1 case of lung metastasis. Postoperative CT examination showed 15 cases of sigmoid colon cancer, 25 cases of ascending colon cancer, and 10 cases of descending colon cancer.With pathologically confirmed, there were 13 cases of sigmoid colon cancer, 27 cases of ascending colon cancer, and 10 cases of descending colon cancer. The coincidence rate of CT diagnosis was 92.0%(46/50). The comparison of preoperative T stage, N stage and pathological results of colon cancer by CT examination showed that the difference was not significant via non-parametric test(P>0.05). Conclusion Multi-slice spiral CT can improve the diagnostic accuracy in the diagnosis of colon cancer, and it has an important reference value for clear preoperative staging.更多还原
