【摘要】 目的利用心理学理论对调配差错进行根因分析,提出改进措施,保障用药安全。方法利用Wichens信息处理模型,从药师信息加工的角度对调配差错进行根因分析。结果药师的调配差错发生在信息接受、处方信息识别、取药指令确定、调配药品和目标药品的信息比对、信息核对5个阶段;调配差错与注意力和记忆2个因素紧密相关。结论心理学理论(Wichens认知模型)可为调配差错的根因分析提供可靠的理论支持,有助于为药学人员提出调配差错的改进措施。更多还原

【Abstract】 Objective To analyze the root cause of dispensing errors by using psychological theory,and propose improvement measures to ensure the safety of medication.Methods The root cause of dispensing errors were analyzed from the perspective of pharmacist information processing by Wichens information processing model.Results The dispensing errors of pharmacists occurred in 5 stages: information acceptance,identification of prescription information,determination of drug taking instructions,information comparison of dispensing drugs and target drugs, and information checking.Dispensing errors were closely related to two factors of attention and memory.Conclusion Psychological theory(Wichens cognitive model) can provide reliable theoretical support for the root cause analysis of dispensing errors.It is useful for the pharmacists to propose improvement measures for dispensing errors.更多还原
