无痛性主动脉夹层的临床特点与多层螺旋CT平扫诊断
Clinical features of painless aortic dissection and the diagnosis of MSCT
【摘要】 目的:分析无痛性主动脉夹层的临床特点,探讨MSCT平扫对无痛性主动脉夹层的早期诊断价值。方法:收集经CT血管造影(CTA)证实为主动脉夹层的患者107例,其中有典型胸背部疼痛者(疼痛组)88例,无典型胸背部疼痛者(无痛组)19例,比较两组间平均年龄、性别、DeBakey分型及MSCT平扫有关征象的差异。结果:疼痛组平均年龄(56.70±16.58),De BakeyⅠ型22例、Ⅱ型10例、Ⅲ型56例,男69例,女19例,CT平扫显示主动脉扩张42例、内膜钙斑内移49例、内膜片29例、管腔密度不均45例;无痛组平均年龄(63.15±19.62),男12例,女7例,DeBakeyⅠ型3例、Ⅱ型5例、Ⅲ型11例,CT平扫显示主动脉扩张14例、内膜钙斑内移9例、内膜片7例、管腔密度不均10例。两组间主动脉扩张征象的差异有统计学意义(P <0.05)。结论:MSCT平扫对于无痛性主动脉夹层的早期诊断具有重要价值,有助于临床及时治疗、降低病死率。
【Abstract】 Objective To analyze the clinical features of painless aortic dissection and explore the early diagnostic value of MSCT non-enhanced scan in painless aortic dissection. Methods 107 patients with aortic dissection diagnosed by CT angiography(CTA) were collected, including 88 patients with typical chest and back pain(pain group) and 19 patients without typical chest and back pain(painless group). The differences in mean age, sex, DeBakey typing and main signs of MSCT scan were compared between the two groups. Results The mean age of pain group was(56.70±16.58),There were 69 males and 19 females, DeBakey Ⅰ type 22 cases, DeBakey Ⅱ Type 10 cases, and DeBakey Ⅲ type 56 cases.CT scan showed aortic dilatation in 42 cases, calcareous plaque inward shift in 49 cases, intimal flap in 29 cases, uneven lumen density in45 cases. The mean age of painless group was(63.15±19.62),including 12 males and 7 females, There were 3 cases of type Ⅰ, 5 cases of type Ⅱ and 11 cases of type Ⅲ. CT scan showed aortic dilatation in 14 cases, calcareous plaque inward shift in 9 cases, intimal flap in 7 cases, uneven lumen density in 10 cases. There were significant differences in the signs of aortic dilatation between the two groups(P < 0.05). Conclusion MSCT scan is of great value in the early diagnosis of painless aortic dissection, and it is helpful to prompt clinical treatment and reduce mortality.
