三维OCT早期诊断开角型青光眼的价值

【摘要】 目的探讨三维光学相干断层扫描技术(OCT)对开角型青光眼早期诊断的价值。方法回顾性分析2016年5月~2019年5月黑龙江省医院收治的86例开角型青光眼患者临床资料,根据病情发展阶段不同分为A组(早期开角型青光眼,43例)、B组(中晚期开角型青光眼,43例),比较两组各部位神经节细胞复合体(GCC)、黄斑区视网膜神经纤维层(mRNFL)、黄斑区神经节细胞层+内丛状层(GCIP)、视盘周围神经纤维(pRNFL)厚度。结果 A组mRNFL、GCIP、GCC上方、下方厚度均大于B组,pRNFL上方、下方、鼻侧、颞侧厚度均大于B组,差异有统计学意义(P<0.05)。结论三维OCT用于开角型青光眼的早期诊断能够较好的呈现出视网膜神经纤维层变薄的范围和区域,且不同病情发展阶段的mRNFL、GCIP、GCC、pRNFL存在较大差异,能够为疾病的评估提供较为明确的依据。

【Abstract】 Objective To explore the value of three-dimensional optical coherence tomography(OCT) in the early diagnosis of open-angle glaucoma.Methods The clinical data of 86 patients with open-angle glaucoma treated in Heilongjiang Provincial Hospital from May 2016 to May 2019 were retrospectively analyzed. They were divided into group A(early open-angle glaucoma, 43 cases) according to the stage of disease development, in group B(43 patients with open-angle glaucoma in the middle and late stages), the ganglion cell complex(GCC), macular retinal nerve fiber layer(mRNFL), macular ganglion cell layer + inner plexiform layer(GCIP), peripheral nerve fiber(pRNFL) thickness. Results The thickness of mRNFL,GCIP, and GCC above and below group A were greater than those of group B, and the thickness of pRNFL above, below, nasal, and temporal were greater than those of group B,the difference was statistically significant(P<0.05). Conclusion Three-dimensional OCT for early diagnosis of openangle glaucoma can better show the range and area of retinal nerve fiber layer thinning, and there are large differences in mRNFL, GCIP, GCC,pRNFL at different stages of disease development, which can be a disease the assessment provided a clearer basis.
