对比增强SWI对不同病理类型肺癌脑转移瘤检出的初步研究

【摘要】 目的:探讨常规对比增强T1WI(CE-T1WI)、磁敏感加权成像(SWI)及增强磁敏感加权成像(CE-SWI)在不同病理类型肺癌脑转移瘤检出率方面的差异。方法:纳入55例经肺部手术或穿刺活检病理证实肺癌患者,其中鳞癌16例、腺癌25例、小细胞癌14例。对全部患者均行颅脑MR常规平扫,包括(T1WI、T2WI、T2FLAIR)、对比增强T1WI及对比增强SWI;转移灶行3个月后随访证实。结果:卡方检验结果显示SWI、CE-T1WI、CE-SWI对鳞癌脑转移瘤检出率差异无统计学意义(P=0.728);对腺癌脑转移瘤检出率三者差异有统计学意义(P<0.001);对小细胞癌脑转移瘤检出率三者有统计学差异(P=0.004)。结论:CE-SWI兼顾SWI和CE-T1WI两者的优点,因此在脑转移瘤MRI检查中CE-SWI是CE-T1WI一个有利补充。

【Abstract】 Purpose: To investigate the differences in the detection of brain metastases from different pathological types of lung cancer by conventional contrast-enhanced T1 WI(CE-T1 WI), magnetic susceptibility-weighted imaging(SWI), and enhanced magnetic susceptibility-weighted imaging(CE-SWI).Methods: Fifty-five patients with lung cancer who underwent operation or lung biopsy were included. Among them, 16 cases were lung squamous cell carcinoma, 25 cases were lung adenocarcinoma, and 14 cases were small cell lung cancer. All patients underwent routine MRI scans of the brain, including(T1 WI, T2 WI, and T2 FLAIR), contrast-enhanced T1 WI, and contrastenhanced SWI. Follow-up observations confirmed metastases after 3 months. Results: Chi-square test results showed no significant difference of detection rate of brain metastases from lung squamous cell carcinoma(P=0.728) between SWI, CE-T1 WI and CE-SWI. There were significant differences of the detection rate of brain metastases from lung adenocarcinoma( P<0.001) between them. The difference of the detection rate of brain metastases from small cell lung cancer was with statistical significant(P=0.004). Conclusion: CE-SWI takes into account the advantages of both SWI and CE-T1 WI, therefore CE-SWI is a favorable complement to CE-T1 WI in MRI examination of brain metastases.
