【摘要】 目的探讨切开复位内固定治疗陈旧性肱骨外髁骨折的疗效。方法回顾性分析2010年7月—2014年6月柳州市工人医院收治的儿童陈旧性肱骨外髁骨折患儿16例,其中男11例,女5例,平均年龄为6.80岁,Jakob分型Ⅱ型9例、Ⅲ型7例,受伤至手术时间为3～10周,平均5.6周;术前肘关节平均活动度为伸(40.00±15.00)°,屈(80.00±10.00)°,前臂旋前(25.00±6.00)°、旋后(30.00±9.00)°。随访时疗效评价包括术后并发症、肘关节屈伸与前臂旋转功能、骨折愈合时间,并以Mayo肘关节功能评分评定术后肘关节功能。结果术后随访时间为4～18个月,平均11个月;骨折愈合时间为8～12周,平均9.7周。术后肘关节平均活动度为伸(10.00±9.00)°、屈(120.00±15.00)°,前臂旋前(70.00±7.00)°、前臂旋后(75.00±8.00)°;术后均无神经血管损伤表现。按照Mayo肘关节功能评分标准评定:优11例,良3例,可2例;优良率为87.50%。结论采用切开复位内固定治疗儿童陈旧性肱骨外髁骨折,能早期功能锻炼,有利于肘关节功能的恢复。更多还原

【Abstract】 Objective To evaluate the therapeutic results of open reduction and internal fixation for neglected lateral humeral condylar fracture in children.Methods A retrospective analysis was conducted for 16 children with neglected lateral humeral condylar fracture cared at Liuzhou Worker’s Hospital from July 2010 to June 2014.There were 11 boys and 5 girls with an average age of 6.8 years old.According to Jakob classification,9 case were typeⅡ,7 cases were typeⅢ.The duration between injured time and operation time ranged from 3 to 10 weeks(average 5.6 weeks).Before operation,the average range of elbow joint extension was(40.00±15.00)°,while that of elbow flexion was(80.00±10.00)°.Range of forearm pronation averaged(25.00±6.00)°,while that of forearm supination averaged(30.00±9.00)°.At follow-up,the treatment results assessment included postoperative complications,the function of elbow joint extension and flexion and of the forearm rotation,and the bone healing time.Postoperative elbow joint function was assessed by using the Mayo Elbow Performance Score.Results All patients were followed up for 4 to 18 months with an average of 11 months.The bone healing time ranged from 8 to 12 weeks(average 9.7 weeks).Postoperative average range of elbow extension was(10.00±9.00)°,while that of elbow flexion was(120.00±15.00)°.The average range of forearm pronation and supination was(70.00±7.00)°and(75.00±8.00)°,respectively.No postoperative nerve and blood vessel injury occurred.According to Mayo Elbow Performance Score,there were excellent in 11 cases,good in 3 cases,fare in 2 cases.The excellent and good rate was 87.50%.Conclusion The treatment of neglected lateral humeral condylar fracture in children with open reduction and internal fixation can result in early functional exercise and be beneficial for the recovery of elbow joint function.更多还原
