【摘要】 目的探讨血清鸢尾素水平与老年2型糖尿病肾病的相关性。方法老年2型糖尿病肾病患者80例,根据肾小球滤过率估值（eGFR）和尿微量白蛋白与肌酐比值（ACR）分为四组,每组20例。A组eGFR≥60 ml·min^-1·1.73 m^-2,ACR≥30 mg/g;B组eGFR≥60 ml·min^-1·1.73 m^-2,ACR<30 mg/g;C组eGFR<60 ml·min^-1·1.73 m^-2,ACR≥30 mg/g;D组eGFR<60 ml·min^-1·1.73 m^-2,ACR<30 mg/g。检测四组血清鸢尾素水平,并采用Pearson线性相关性分析其与相关临床指标的相关性,逐步多元线性回归分析血清鸢尾素水平的影响因素。结果 B组ACR水平低于A组,鸢尾素水平高于A组（P<0.05）。C组鸢尾素水平低于A、B、D组（P<0.05）,而D组鸢尾素水平仅低于B组（P<0.05）。Pearson相关分析显示,血清鸢尾素水平与BUN、SCr、尿酸和ACR呈负相关（r=-0.448、-0.624、-0.266和-0.605,P<0.05）,与eGFR呈正相关（r=0.636,P<0.05）。逐步多元线性回归分析显示,ACR和eGFR是鸢尾素水平的独立影响因素（P<0.05）。结论在老年2型糖尿病肾病患者,随着eGFR下降和ACR增加,血清鸢尾素显著下降;血清鸢尾素可能参与了糖尿病肾病的发展过程。更多还原

【Abstract】 Objective To explore the correlation between serum irisin level and type 2 diabetic nephropathy（DN） in elderly patients.Methods According to glomerular filtration rate（eGFR） and the ratio of urinary albumin to creatinine（ACR）,80 elderly patients with DN were divided into four groups with 20 cases each.The patients in group A were with eGFR≥60 ml·min^-1·1.73 m^-2 and ACR≥30 mg/g,those in group B were with eGFR≥60 ml·min^-1·1.73 m^-2 and ACR<30 mg/g,those in group C were with eGFR<60 ml·min^-1·1.73 m^-2 and ACR≥30 mg/g,and those in group D were with eGFR<60 ml·min^-1·1.73 m^-2 and ACR<30 mg/g.Serum levels of irisin were detected and the correlation of irisin and relevant clinical indicators was analyzed.The factors affecting serum irisin level were analyzed by stepwise multiple linear regression.Results The ACR was lower and irisin was higher in group B than those in group A（P<0.05）.Serum level of irisin in group C was lower than that in groups of A,B and D（P<0.05）,which was lower in group D than that in group B（P<0.05）.Pearson correlation analysis showed that serum irisin level was negatively correlated with BUN,SCr,UA and ACR（r=-0.448,-0.624,-0.266 and-0.605,P<0.05）,and positively correlated with eGFR（r=0.636,P<0.05）.Stepwise multiple linear regression analysis showed that ACR and eGFR were the independent factors affecting serum irisin level（P<0.05）.Conclusion In elderly patients with DN,as with the decrease of eGFR and the increase of ACR,serum irisin level is decreased significantly,which may be involved in the development of DN in elderly T2 DM patients.更多还原
