新生儿黄疸对血清心肌酶水平的影响

【摘要】 目的研究新生儿黄疸对心肌酶水平的影响。方法选取2016年4月至2018年11月在寿光市人民医院住院的186例黄疸新生儿,分为生理性黄疸组(89例)和病理性黄疸组(97例)。另选取同期在寿光市人民医院出生的92例足月无黄疸健康新生儿为对照组。比较3组新生儿血清肌酸激酶(CK)、肌酸激酶同工酶MB(CK-MB)、乳酸脱氢酶(LDH)水平,轻中度与重度病理性黄疸患儿血清CK、CK-MB、LDH水平,病理性黄疸组患儿治疗前后血清CK、CK-MB、LDH水平。结果生理性黄疸组和病理性黄疸组新生儿血清CK、CK-MB、LDH水平高于对照组,病理性黄疸组新生儿血清CK、CK-MB、LDH水平高于生理性黄疸组,差异有统计学意义(均P<0.05)。重度病理性黄疸组患儿血清CK、CK-MB、LDH水平高于轻中度病理性黄疸患儿,差异有统计学意义(均P<0.05)。治疗后,病理性黄疸患儿血清CK、CK-MB、LDH水平均较治疗前降低,差异有统计学意义(均P<0.05)。结论黄疸新生儿心肌酶谱明显升高,病理性黄疸新生儿心肌酶谱升高更显著,应加强相关检测及监护,及早干预,避免永久性心肌损伤的发生。

【Abstract】 Objective To study the effects of neonatal jaundice on serum myocardial enzyme levels.Methods A total of 186 neonates with jaundice admitted to Shouguang People’s Hospital from April 2016 to November 2018 were enrolled, and divided into physiological jaundice group(89 cases) and pathological jaundice group(97 cases). Another 92 healthy infants born without jaundice in Shouguang People’s Hospital at the same time were enrolled as control group. The serum levels of creatine kinase(CK), creatine kinase-MB(CK-MB) and lactic dehydrogenase(LDH) in the three groups were compared. The levels of serum CK, CK-MB and LDH in children with mild to moderate and severe pathological jaundice were compared. Serum levels of CK, CK-MB and LDH were compared before and after treatment in pathological jaundice group.Results Serum levels of CK, CK-MB and LDH in physiologic jaundice group and pathological jaundice group were higher than those in control group, and the differences were statistically significant(all P<0.05). The levels of serum CK, CK-MB and LDH in pathological jaundice group were higher than those in physiological jaundice group, and the differences were statistically significant(all P<0.05). The serum levels of CK, CK-MB and LDH in children with severe pathological jaundice were higher than those in children with mild to moderate pathological jaundice, and the differences were statistically significant(all P<0.05). After treatment, the serum levels of CK, CK-MB and LDH in pathological jaundice group were lower than those before, and the differences were statistically significant(all P<0.05).Conclusion The myocardial zymogram of jaundiced neonates could significantly increase,and those of neonates with severe pathological jaundice increase more significantly. It is necessary to strengthen related detection and monitoring, and intervene as early as possible to avoid permanent myocardial damage.
