128层双源CT改良回顾性心电门控与非门控扫描在支气管动脉成像中的应用

【摘要】 目的探讨128层双源CT改良回顾性心电门控序列扫描与普通增强扫描在支气管动脉显示及图像质量方面的临床应用价值。方法收集200例患者,按照扫描方法将其分为两组,100例改良回顾性心电门控序列扫描患者,100例胸部普通增强患者。观察两组支气管动脉的清晰显示率并测量支气管动脉的信噪比(SNR)、对比噪声比(CNR)并由两名放射科医生主观评价其图像质量。结果心电门控组支气管动脉纵膈段和肺门段清晰显示率分别为100%和86. 92%,普通增强组支气管动脉纵膈段和肺门段清晰显示率分别为93. 46%和57. 94%,心电门控组纵膈段和肺门段支气管动脉清晰显示率均高于普通增强组。心电门控组与普通增强组支气管动脉的SNR分别为31. 93±8. 85、18. 99±4. 56,CNR分别为25. 66±8. 05、14. 51±4. 65,主观评分分别为4. 30±0. 57、3. 68±0. 57,主观评分、SNR、CNR内两组数据两两比较均有统计学意义(P值均<0. 05),心电门控组SNR、CNR及主观评分均高于普通增强组。结论 128层双源CT改良回顾性心电门控序列扫描获得的支气管动脉图像无论在显示率还是图像质量方面均优于普通增强扫描组,因此,针对支气管动脉扫描,可以选用改良回顾性心电门控序列扫描,获得更加优质的图像,有效地服务于临床。

【Abstract】 Objective To investigate the clinical value of 128-segment dual-source CT retrospective ECG gated sequence scan and common enhancement scan in bronchial artery display and image quality. Methods We conducted a retrospective analysis of100 patients with improved retrospective ECG gating sequence scanning and 100 patients with common chest enhancement,whowere divided into two groups according to the scanning method. The clear display rate of the bronchial arteries was observed andthe signal-noise ratio( SNR) and contrast noise ratio( CNR) of the bronchial arteries were measured and their image quality wassubjectively evaluated by two radiologists. Results The clear display rate of the bronchial artery longitudinal segment and the hi-lar segment in the ECG-gated group was 100% and 86. 92%,respectively. The clear display rates of the bronchial artery longitu-dinal segment and the hilar segment were 93. 46% and 57. 94%,respectively. The clear display rate of the bronchial artery in thelongitudinal segment and the hilar segment of the electric gated group was higher than that in the normal enhanced group. TheSNR of the bronchial artery in the ECG-gated group and the normal-enhanced group were 31. 93 ± 8. 85 and 18. 99 ± 4. 56,re-spectively,and the CNR was 25. 66 ± 8. 05 and 14. 51 ± 4. 65,respectively. The subjective scores were 4. 30 ± 0. 57 and 3. 68 ±0. 57,respectively. There were statistically significant differences between the two groups in the CNR( P < 0. 05). The SNR,CNR and subjective scores of the ECG-gated group were higher than those of the general enhancement group. Conclusion Thebronchial artery image obtained by the 128-segment dual-source CT improved retrospective ECG gating sequence scan is superior tothe general enhanced scan group in both display rate and image quality. Therefore,for bronchial artery scanning,we can use the improved retrospective ECG. Control sequence scanning can obtain better quality images and effectively serve the clinic.
