【摘要】 研究表明青光眼手术会加速白内障的发生、发展。青光眼术后的患者眼部结构具有一定的特殊性,如滤过泡的存在、角膜内皮细胞计数低、浅前房、虹膜后粘连、瞳孔变形、晶状体核硬度大及悬韧带损伤等,这增加了白内障手术的难度和风险,对于眼科医师来讲是很大的挑战。临床医师应详细评估青光眼术后白内障患者术前眼部情况,正确选择手术时间及手术方案,减少术中不利于滤过泡的操作,术后积极抗炎对症治疗并适当延长糖皮质激素类药物的使用,使患者视力恢复的同时,尽量减少眼压升高的风险,提高患者的生活质量。该文对青光眼手术加速白内障的发生、发展及抗青光眼术后白内障手术难点进行了综述,以期为临床医师提供参考。更多还原

【Abstract】 Previous studies have shown that glaucoma surgery will accelerate the incidence and development of cataract. The ocular structure of patients undergoing glaucoma surgery has the following characteristics including the presence of filtering bleb, low corneal endothelial cell count, shallow anterior chamber, posterior synechiae, pupillary deformation, hard lens nucleus and suspensory ligament injury, etc., which increase the difficulty and risk of cataract surgery and pose a huge challenge for ophthalmologists. Clinicians should explicitly evaluate the preoperative ocular condition of cataract patients undergoing glaucoma surgery, select the optimal date of surgery and surgical regime, reduce intraoperative procedures that are not conducive to filtering bleb, actively deliver anti-inflammatory treatment and prolong the use of glucocorticoids after cataract surgery, aiming to restore the visual acuity, minimize the risk of elevated intraocular pressure and enhance the quality of life of patients. In this article, the role of glaucoma surgery in accelerating the incidence and progression of cataract and the surgical difficulty of cataract surgery after glaucoma surgery was reviewed, aiming to provide reference for clinicians.更多还原
