血管生成拟态及骨桥蛋白和基质金属蛋白酶2在三阴性乳腺癌组织中的表达及意义
Expression and significance of vasculogenic mimicry,osteopontin and matrix metalloproteinase-2 in triple-negative breast cancer
【摘要】 目的探讨三阴性乳腺癌(TNBC)组织中血管生成拟态(VM)、骨桥蛋白(OPN)及基质金属蛋白酶2(MMP-2)的表达及意义。方法收集2012年1月至2015年12月南阳市中心医院收治的120例首次经病理检查确诊的女性TNBC患者的癌组织标本为观察对象,另收集同期30例女性良性乳腺增生组织标本作为对照组;采用CD31及过碘酸-希夫液双重染色法检测TNBC及良性乳腺增生组织中VM的表达,采用免疫组织化学染色检测TNBC及良性乳腺增生组织中OPN、MMP-2的表达,并分析其与TNBC临床病理特征的关系,以及TNBC组织中VM、OPN、MMP-2表达的相关性。结果 TNBC组织中VM、OPN、MMP-2阳性表达分别为32例(26.7%)、30例(25.0%)、86例(71.7%),良性乳腺增生组织中VM、OPN、MMP-2阳性表达分别为0例(0.0%)、1例(3.3%)、5例(16.7%);TNBC组织中VM、OPN、MMP-2阳性表达率显著高于良性乳腺增生组织(χ~2=10.169、6.872、30.425,P<0.05)。VM、OPN及MMP-2在TNBC组织中的表达与肿瘤大小、淋巴结转移情况和临床分期有关(P<0.05),与患者年龄、绝经状态及家族史无关(P>0.05)。TNBC组织中VM与OPN、MMP-2的表达呈正相关(r=0.609、0.296,P<0.05),OPN与MMP-2的表达呈正相关(r=0.192,P<0.05)。结论 VM、OPN及MMP-2在TNBC组织中高表达,并在其发生、发展及侵袭转移的过程中起协同作用。同时检测三者表达情况有助于判断TNBC的增殖转移特征,对TNBC的治疗及预后判断提供一定的理论依据。
【Abstract】 Objective To investigate the expression and significance of vasculogenic mimicry( VM),osteopontin( OPN) and matrix metalloproteinase-2( MMP-2) in triple-negative breast cancer( TNBC) tissues. Methods The breast cancer tissue specimens of 120 female patients with TNBC who were the first diagnosed by pathological examination in Nanyang Central Hospital from January 2012 to December 2015 were collected as the research subjects. The benign breast hyperplasia specimens of 30 women were collected as control group in the same period. The expression of VM in TNBC tissues and benign breast hyperplasia tissues was detected by CD31 and periodic acid-Schiff double staining. The expression of OPN and MMP-2 in TNBC tissues and benign breast hyperplasia tissues was detected by immunohistochemical staining. The relationship between the expression of VM and OPN and MMP-2 and the clinicopathological features of TNBC was analyze. The correlation of the expression of VM,OPN and MMP-2 in TNBC tissues was analyzed. Results The positive expression rate of VM,OPN and MMP-2 in TNBC tissues was 26. 7%( 32/120),25. 0%( 30/120) and 71. 7%( 86/120),respectively. The positive expression rate of VM,OPN and MMP-2 in benign breast hyperplasia tissues was 0. 0%( 0/30),3. 3%( 1/30) and 16. 7%( 5/30),respectively. The positive rates of VM,OPN and MMP-2 in TNBC tissues were significantly higher than those in benign breast hyperplasia tissues( χ~2= 10. 169,6. 872,30. 425; P < 0. 05). The expression of VM,OPN and MMP-2 was correlated with the tumor size,lymph node metastasis and clinical stage in TNBC patients( P < 0. 05),but it was not related to the age,menopausal state and family history of patients( P > 0. 05). The expression of VM was positively correlated with the expression of OPN and MMP-2 in breast cancer tissues( χ~2= 0. 609,0. 296; P < 0. 05),and the expression of OPN was positively correlated with the expression of MMP-2 in breast cancer tissues( r = 0. 192,P < 0. 05). Conclusion The expression of VM,OPN and MMP-2 are up-regulated in TNBC tissues,and play a synergistic role in the occurrence,development and invasion and metastasis of TNBC. The combined detection of the three factors is helpful to judge the proliferation and metastasis of TNBC,and provides a theoretical basis for the treatment and prognosis of TNBC.
