【摘要】 目的探讨不同分期梅毒患者外周血中T细胞亚群表达差异及价值。方法选取2017年1月至2019年5月四川省武胜县人民医院诊治的120例不同时期梅毒患者作为研究对象。并按照不同梅毒分期分为三组,梅毒Ⅰ期患者为A组(40例)、梅毒Ⅱ期患者为B组(41例)和梅毒Ⅲ期患者为C组(39例)。检测三组外周血中T细胞亚群CD3~+、CD4~+、CD4+T和Th1水平以及IL-6、IL-8、IL-17和TNF-α表达,并进行不同梅毒分期外周血中T细胞亚群指标之间的TRUST滴度对比。结果①A组年龄(35.17±8.26)岁、B组年龄(36.42±9.43)岁、C组年龄(35.36±7.68)岁,三组之间年龄差异无统计学意义(P>0.05)。②不同时期梅毒患者CD3~+、CD4~+、CD4+T和Th1表达:与A组相比,B组和C组的CD3~+和CD4~+显著增高;B组和C组的CD4+T和Th1显著下降。不同时期的外周血T淋巴细胞亚群之间差异均具有统计学意义(均P<0.05)。③不同时期梅毒患者CD3~+、CD4~+、CD4+T和Th1的TRUST滴度对比结果显示:与A组相比,B组和C组的CD3+和CD4~+的TRUST滴度TRUST滴度显著增高;B组和C组的CD4+T和Th1的TRUST滴度显著下降。不同时期的外周血T淋巴细胞亚群指标滴度之间差异均具有统计学意义(均P<0.05)。④不同时期梅毒患者IL-6、IL-8、IL-17和TNF-α表达结果显示:与A组相比,B组和C组的IL-6、IL-8、IL-17和TNF-α均显著升高,且差异均具有统计学意义(均P<0.05)。⑤不同时期梅毒患者IL-6、IL-8、IL-17和TNF-α的TRUST滴度对比结果显示:与A组相比,B组和C组的IL-6、IL-8、IL-17和TNF-α的TRUST滴度显著升高且差异具有统计学意义(P<0.05)。结论不同分期的梅毒患者的外周血T淋巴细胞亚群CD3+、CD4+/IL-6、IL-8、IL-17和TNF-α表达会随着病情严重程度其表达量升高,但CD4+T和Th1的表达会随着病情严重程度而减少,因此对梅毒患者进行上述指标的检测价值较高。更多还原

【Abstract】 Objective To evaluate the expression differences and value of T cell subsets in peripheral blood of syphilis patients at different stages. Methods 120 syphilis patients at different stages in People′s Hospital of Wusheng County between January 2017 and May 2019 were selected and according to the different stages, were divided into group A(stage I, n=40), group B( stage II, n=41), and group C(stage III, n=39). The levels of CD3+, CD4+, CD4+T and Th1, as well as the expressions of IL-6, IL-8, IL-17 and TNF-α in the T-cell subsets in peripheral blood of the three groups were detected, and the TRUST titers of the T-cell subsets in peripheral blood of patients at different syphilis stages were compared(P>0.05). Results ① The age of group A, group B and group C was(35.17±8.26),(36.42±9.43) and(35.36±7.68) years old, without statistically signifant differences(P>0.05). ② In terms of the expressions of CD3+, CD4+, CD4+T and Th1 in syphilis patients at different stages, CD3+ and CD4+ in group B and group C were significantly increased compared with that in group A and the CD4+T and Th1 were significantly decreased in group B and group C, all with statistically significant differences(all P< 0.05). ③ The TRUST titers of CD3+, CD4+, CD4+T and Th1 in syphilis patients at different stages showed that, compared with group A, the TRUST titers of CD3+ and CD4+ in group B and group C were significantly increased; CD4+T and Th1 TRUST titers in group B and C were significantly decreased, all with statistically signifant differences(all P <0.05). ④ The expression of IL-6, IL-8, IL-17 and TNF-α in syphilis patients at different stages showed that, compared with group A, IL-6, IL-8, IL-17 and TNF-α in group B and group C were significantly increased with statistically significant differences(all P<0.05). ⑤ Comparison of the TRUST titers of IL-6, IL-8, IL-17 and TNF-α in patients with syphilis at different stages showed that, compared with group A, the TRUST titers of IL-6, IL-8, IL-17 and TNF-α in group B and group C were significantly increased, with statistically significant difference(P<0.05). Conclusions The expression levels of CD3+, CD4+/IL-6, IL-8, IL-17 and TNF-α in peripheral blood T-lymphocyte subsets of syphilis patients at different stages increase with the increasing severity of the disease, but the expression levels of CD4+T and Th1 decrease with the increasing severity of the disease, so the above indicators are of high value in the detection of syphilis patients.更多还原
