CXCR4和CD26在肺浸润性腺癌中的表达及意义
Expression and significance of CD26 and CXCR4 in invasive adenocarcinoma of the lung
【摘要】 目的观察并分析CXCR4和CD26在浸润性肺腺癌原发肿瘤组织以及相关淋巴结的转移性肿瘤组织中的表达及意义,分析观察结果,探讨两者的相关性,以及与肺浸润性腺癌各种临床病理特征、预后的联系。方法采用免疫组织化学法检测肿瘤旁正常肺组织57例、肺浸润性腺癌原发肿瘤组织222例以及在淋巴结中的转移性肿瘤组织54例CXCR4和CD26的表达。结果 CXCR4蛋白的阳性表达率在肺浸润性腺癌癌组织中的表达率高于癌旁正常肺组织中的表达率,并且其表达率低于在淋巴结中的转移癌组织中的表达率,差异具有统计学意义(P<0.05);CD26蛋白的阳性表达率在肺浸润性腺癌癌组织中的表达率高于癌旁正常肺组织中的表达率,并且其表达率低于在淋巴结中的转移癌组织中的表达率,差异具有统计学意义(P<0.05);CXCR4在222例肺浸润性腺癌原发癌组织中的表达水平与浸润性肺腺癌形态学分级、淋巴结有或者没有癌组织转移和临床TNM分期相关(P<0.05),CD26在222例肺浸润性腺癌原发癌组织中的表达水平与肺浸润性腺癌患者肿瘤的形态学分级及大小相关(P<0.05)。Kaplan-Meier生存分析说明在P<0.05情况下,肺浸润性腺癌患者的性别、年龄、肿瘤大小、淋巴结有或者没有癌组织转移、临床TNM分期为影响患者术后的生存时间的有效因素(P<0.05); Cox回归模型对实验结果进行分析后显示,在P<0.05情况下,以下情况成为影响肺浸润性腺癌患者预后好坏的独立危险因素:肺浸润性腺癌患者的年龄、瘤体体积、部分肿瘤的形态学分级、淋巴结有或者没有癌组织转移、临床TNM分期(P<0.05)。Spearman等级相关分析提示CXCR4和CD26在肺浸润性腺癌原发肿瘤组织中的表达与患者的性别、年龄、形态学分级、临床TNM分期、肿瘤大小和淋巴结有或者没有淋癌组织转移呈正相关(r>0,P<0.05)。结论 CD26和CXCR4在肺浸润性腺癌的发生、发展中具有重要意义,有助于评估浸润性腺癌患者的预后好坏。
【Abstract】 Objective The relevant research has indicated that chemokine receptor 4(CXCR4) closely related to tumor development. It can promote cell growth and inhibit apoptosis and other important functions, and has been confirmed can promote metastasis in a variety of tumors. Peptide peptidase IV(DPP IV), also known as CD26, is involved in protein hydrolysis, cell adhesion, immune response, regulation of apoptosis, and plays an important role in tumor invasion and metastasis. The purpose of this study was to analyze the expression characteristics of CXCR4 and CD26 in the invasive adenocarcincoma of the lung and lymph node metastases, and to investigate the relationship between the expression of CXCR4 and CD26 and the clinicopathological features and prognosis of invasive adenocarcincoma of the lung. Methods The tissue chip and immunohistochemistry techniques were used to detect the expression of CXCR4 and CD26 in 57 cases of normal lung tissues, 222 cases of the primary lesion of invasive adenocarcinoma of the lung and 54 cases of corresponding lymph node metastases. Results The expression of CXCR4 in adjacent normal lung tissues, the primary lesion of the invasive adenocarcinoma of the lung, and lymph node metastases was gradually increased(P<0.05), and the expression of CD26 in adjacent normal lung tissues, the primary tumor of the invasive adenocarcinoma of the lung, and lymph node metastases appeared upward tendency(P<0.05). In addition, the expression level of CXCR4 in the primary lesion was relevant to morphological grading, metastases and TNM staging of invasive adenocarcinoma of the lung(P<0.05), as well as the expression level of CD26 in the primary lesion was correlated with morphological grading and the tumor size of invasive adenocarcinoma of the lung(P<0.05). Kaplan-Meier survival analysis relevant that gender, age, tumor size, morphological grading, metastasis, TNM staging were correlated with the postoperative survival rate of invasive adenocarcinoma of the lung patients(P<0.05). The Cox regression model was used to determine gender, age, morphological grading, tumor size, metastasis and TNM staging were independent prognostic factors of invasive adenocarcinoma of the lung(P<0.05). Spearman correlation analysis showed that the expression of CXCR4 and CD26 in invasive adenocarcinoma of the lung was positively correlated with independent prognostic factors of invasive adenocarcinoma of the lung(r>0,P<0.05). Conclusion CXCR4 and CD26 may play important roles in invasion and metastasis in invasive adenocarcinoma of the lung, so these tow marker can be employed to evaluate the prognosis of the invasive adenocarcinoma of the lung patients.
