【摘要】 目的:探讨肌骨超声检测幼年特发性关节炎关节病变(Juvenile Idiopathic Arthritis,JIA)的超声表现,为JIA的超声检测提供参考依据。方法:回顾性的方法纳入我院2015年4月至2017年8月我院确诊为JIA患儿共计148例,采用肌骨超声(Musculoskeletal ultrasound,MSUS)和MRI、MRI增强检查,评估双侧近端指间关节、肘、腕、肩、掌指关节、髋、膝、踝关节和跖趾关节的检查情况。并收集体格检查的结果。结果:共计138例患儿超声检查结果表现为阳性,1个以上关节受累的患儿共计74例,关节腔积液110例,滑膜增厚76例,肌腱改变92例,软骨改变16例,滑膜彩色多普勒信号增多50例,骨侵蚀:8例,滑囊炎4例。所有病例中有2例MRI与肌骨超声结果有所差异,而所有病例的增强MRI与肌骨超声结果均一致。结论:肌骨骼超声对幼年特发性关节炎关节病变检测可取得良好的灵敏度,且检测方便,可能可以作为诊断JIA的检测办法。更多还原

【Abstract】 Objective:To investigate ultrasonography finding of musculoskeletal ultrasound(MSUS) in the detection of Juvenile Idiopathic Arthritis(JIA) and to provide the ultrasound diagnosis reference of JIA.Methods:A retrospective study of a total of 148 cases with JIA was performed in our hospital from April 2016 to August 2017. MSUS, MRI and enhanced MRI were applied to assess both sides Near the interphalangeal joint, elbow, wrist, shoulder, metacarpophalangeal joint, hip, knee, ankle and metatarsophalangeal joints. The physical examination results were also collected.Results:A total of 138 cases of children showed positive MSUS results. Among these patients, 74 cases demonstrated the involvement of more than one joint. It was shown synovial effusion in 110 cases, synovial thickening in 76 cases, tendon changes in 92 cases, cartilage changes in 16 cases synovial energy Doppler signal increase in 50 cases, bone erosion in 8 cases and bursitis in 4 cases. Among all the cases, 2 cases demonstrated different results between MRI and MSUS, however, the results of enhanced MRI were consistent with MSUS in all the cases.Conclusion:Musculoskeletal ultrasound can be used to detect juvenile idiopathic arthritis with good sensitivity and might be used as the clinical evaluation method.更多还原
