骶尾部入路并VSD引流术治疗骶尾部感染性疾病疗效分析

【摘要】 目的探讨经骶尾部入路手术结合VSD引流术治疗骶尾部感染性疾病的临床疗效。方法 2016年1月~2017年12月我院收治骶尾部感染性疾病患者28例,其中骶尾部囊肿合并感染12例,骶尾部藏毛窦感染16例,感染病灶最大直径约4.5~14.2 cm,平均(7.4±2.2)cm。随机分为两组:实验组14例,行Kraske术并VSD引流术;对照组14例,行Kraske术并常规负压引流。结果两组均顺利完成手术痊愈出院。组间比较,实验组负压引流时间短,术后3 d和7 d的疼痛评分低(P<0.05),而两组手术时间、术中出血量、住院时间及术后满意度比较均无统计学差异(P>0.05)。1年后随访两组均未出现感染灶复发、伤口未愈及异常神经体征。结论改良Kraske术并VSD引流治疗骶尾部感染性疾病,可明显减轻患者术后疼痛,促进愈合,值得临床选择应用。

【Abstract】 Objective To explore the clinical effect of transsacrococcygeal approach Kraske surgery combined with vacuum sealing drainage(VSD) in the treatment of sacrococcygeal infectious diseases. Methods 28 cases with infectious sacrococcygeal pilonidal sinus and cyst from January 2016 to December 2017 were enrolled,including 12 cases of sacrococcygeal pilonidal sinus infection and 16 cases of infectious sacrococcygeal cyst. All cases were performed Krake surgery and randomly divided into two groups, with 14 cases for each: VAD was performed in the experimental group and conventional negative pressure drainage was used in the control group. Results All cases achieved cure and discharged successfully. The duration of drainage was shorter and the VAS scores of both 3 days and 7 days were less in the experimental group than those in the control group(P<0.05). There was no significant difference in operation time, intra-operative blood loss, hospital stays and post-operative satisfaction rate between the two groups(P>0.05). One year later, no recurrence of infection, wound healing and abnormal nerve signs were found in both groups. Conclusion The modified Kraske surgery combined with VSD can reduce postoperative pain significantly and promote healing for sacrococcygeal infectious diseases, which is worthy of clinical popularization.
