CT引导下Hook-wire定位在肺结节胸腔镜手术中的作用
The Role of Preoperative CT-guided Hook-wire Localization for Pulmonary Nodules in Thoracoscopic Resection
【摘要】 目的探究在CT引导下Hook-wire定位对肺结节胸腔镜手术的价值和意义。方法回顾性收集2017年6月~2019年4月期间在我院行肺结节胸腔镜手术的22例患者资料,所有患者在术前行CT引导下Hook-wire定位,再行电视胸腔镜下肺结节部位楔形切除术,切除标本送快速冰冻切片病理分析。统计结节定位的准确率、切除准确率、术后并发症发生率等。结果①CT引导下Hook-wire肺结节定位的准确率为100%。肺结节胸腔镜切除术成功率为100%,结节平均直径为(15.07±3.16)mm,平均定位时间为(15.88±1.25)min;②有1例患者出现了轻微气胸(4.5%),但无需进行额外治疗;未出现中等量气胸、对侧气胸、血胸等并发症,且所有患者在手术过程中未发生定位针脱落和移位。结论 CT引导下Hook-wire肺结节定位在胸腔镜切除术中具有定位准确率高、切除准确率高、定位时间短和术后并发症发生率低的特点,值得在肺结节胸腔镜手术中推广使用。
【Abstract】 Objective To investigate the value and significance of preoperative CT-guided Hook-wire localization for pulmonary nodules in thoracoscopic resection. Methods A retrospective study of 22 patients who underwent thoracoscopic resection of pulmonary nodules in our hospital from June to April 2019 was performed. All patients underwent CT-guided Hook-wire for lung nodules. Then, the thoracic resection of the lung nodules was performed under the video-assisted thoracoscopic surgery. The resected specimens were sent for pathological analysis by rapid frozen sections, and the results were statistically analyzed. Then, the accuracy of the nodule localization, the accuracy of resection, and the postoperative complications were summarized.Results The accuracy of CT-guided Hook-wire lung nodule localization was 100%. The lung nodules were(15.07±3.16)mm in average. The success rate of thoracoscopic resection of the lung nodules was 100%, and the average positioning time was(15.88±1.25) min. One patient developed mild pneumothorax, but no additional treatment was needed. No equal pneumothorax, contralateral pneumothorax, hemothorax, nor detached and displaced positioning needles were found. Conclusions CT-guided Hook-wire pulmonary nodule localization has the characteristics of high accuracy, high resection accuracy, low postoperative complication rate and short positioning time. It is worthy of promotion in thoracoscopic resection.
