人乳头瘤病毒E6/E7 mRNA检测在宫颈病变筛查中的应用进展

【摘要】 目的研究人乳头瘤病毒(HPV)E6/E7 mRNA检测在宫颈病变筛查中的应用进展。方法阐述HPV基本定义以及致癌基因E6和E7对人体免疫功能和宫颈癌浸润转移的影响。分析HPV E6/E7 mRNA检测的基本原理和在宫颈病变筛查中的应用价值,并分析传统HPV E6/E7 mRNA检测法存在的弊端,分析Aptima HPV检测方法和PreTect HPV检测法的原理、应用范围以及价值。结果临床可根据致癌基因E6和E7在宫颈组织中的表达情况,判断该病毒是否具有致癌的活性。对稳定性较差的单链RNA,采用传统的HPV E6/E7 mRNA检测方法,存在较高的假阴性率。Aptima HPV检测法以截点S/CO≥1 Copy·mL-1判定为阳性,其准确度、特异度和敏感度均较高。PreTect HPV检测法仅适用于对科研或个别人群进行检测,不适用大范围的HPV筛查。结论 Aptima HPV检测法能提高检测的准确率和特异度,降低了假阳性的发生,同时减少了不必要的医疗资源浪费,PreTect HPV检测法适用于科研或者个别人群进行检测研究。
【Abstract】 Objective To study the application of human papillomavirus(HPV) E6/E7 mRNA in cervical lesion screening.Methods The basic definition of HPV and the effects of oncogenes E6 and E7 on human immune function and invasion and metastasis of cervical cancer were expounded.The basic principle of HPV E6 and E7 mRNA detection and its application value in cervical lesion screening were analyzed.The principle,application scope and value of Aptima HPV detection method and PreTect HPV detection method were analyzed.Results According to the expression of the oncogene E6 and E7 in cervix tissues,we can judge whether the virus has carcinogenic activity or not.For the single stranded RNA with poor stability,there is a high false negative rate.Aptima HPV detection method is characterized by high accuracy and specificity by using S/CO≥1 Copy·mL-1 as the cut-off point for the detection of single strand RNAs with low stability.PreTect HPV detection is the only suitable method for scientific research or individual population detection,but not for a wide range of HPV screening.Conclusion Aptima HPV detection method can improve the accuracy and specificity of detection,reduce the occurrence of false positive,and reduce the unnecessary waste of medical resources.PreTect HPV detection method is suitable for scientific research or individual population.
