急性左心衰合并肺部感染患者心房利钠肽前体A基因多态性和炎性标志物水平分析

【摘要】 目的:分析急性左心衰合并肺部感染患者心房利钠肽前体A (NPPA)基因多态性分布和炎性标志物水平变化。方法:选择本院收治的急性左心衰患者120例,按其是否合并肺部感染分为左心衰感染组和左心衰组,各60例,以及同期体检的非左心衰或非左心衰合并感染人群60例为对照组。收集各组外周静脉血,采用基因测序仪检测NPPA不同基因型分布;采用免疫发光和免疫散射比浊法分别测定左心衰感染组NPPA的rs5063、rs5065不同基因型患者降钙素原(PCT)和C-反应蛋白(CRP)水平。结果:三组间NPPA基因rs5063、rs5065的AA、GG基因型分布有明显差异(P<0.05或P<0.01),而AG基因型三组间分布无明显差异(P>0.05)。左心衰组和左心衰感染组的AA基因型患者比率明显高于对照组(P<0.01),GG基因型患者比率明显低于对照组(P<0.01);左心衰组与左心衰感染组之间rs5063、rs5065的AA、AG、GG基因型比率差异无统计学意义(P>0.05)。左心衰感染组患者NPPA基因(rs5063、rs5065)的AA、AG、GG基因型患者血清PCT、CRP水平差异均无统计学意义(P>0.05)。结论:NPPA的rs5063和rs5065基因型可能与急性左心衰易感性有关。

【Abstract】 Objective:To explore natriuretic peptide precursor A(NPPA)polymorphisms and inflammatory markers level in patients with acute left heart failure complicated with pulmonary infection.Method:120 patients with acute left heart failure admitted to our hospital were divided into left heart failure infection group and left heart failure group according to whether they were combined with pulmonary infection or not,and 60 patients with nonleft heart failure or left heart failure combined with infection during the same period of physical examination were taken as the control group.Peripheral venous blood of each group was collected,and the distribution of different genotypes of NPPA was detected by gene sequencing.Levels of procalcitonin(PCT)and c-reactive protein(CRP)in NPPA of left heart failure infection group were determined by immunolight and immunoscattering turbidimetry.Results:There were significant differences in the distribution of rs5063,rs5065,AA and GG genotypes among the three groups(P<0.05),while there was no significant difference in the distribution of AG genotype among the three groups(P>0.05).The ratio of AA genotype patients in left heart failure group and left heart failure infection group was significantly higher than that in control group(P<0.01),and the ratio of GG genotype patients was significantly lower than that in control group(P<0.01).There was no significant difference in AA,AG and GG genotypes of rs5063 and rs5065 between left heart failure group and left heart failure infection group(P>0.05).There were no significant differences in serum PCT and CRP levels between AA,AG and GG genotypes of NPPA gene(rs5063 and rs5065)in left heart failure infection group(P>0.05).Conclusion:The genotypes of rs5063 and rs5065 of NPPA may be related to the susceptibility of acute left heart failure.
