超声联合多项血液指标检测对卵巢恶性肿瘤的诊断价值
The Diagnostic Value Of Ultrasonography Combined With Multiple Blood Indexes for Ovarian Malignant Tumor
【摘要】 目的探讨超声联合多项血液指标检测对卵巢恶性肿瘤的诊断价值。方法以2015年10月—2018年10月经手术治疗后病理检验确诊为卵巢肿瘤的80例患者,以及同期经本院体检中心体检的40例健康者作为研究对象,根据诊断结果分为良性肿瘤组、恶性肿瘤组。对两组病例的血清CA125、HE4、ROMA值以及超声检查结果进行收集整理,采用统计学方法对单一指标及联合检测在早期卵巢恶性肿瘤诊断中的价值进行比较分析。结果单一指标诊断良恶性肿瘤方面,CA125、ROMA的灵敏度最高为87.50%;HE4和超声的特异性较高,分别为96.88%和90.63%;阳性预测值方面HE4最高可达83.33%;阴性预测值各指标均在91%以上,差异并不明显。Youden指数方面,CA125与超声差异不大,而HE4、ROMA相对较低。超声+ROMA诊断良恶性卵巢癌的灵敏度、特异度、阳性预测值、阴性预测值及Youden指数分别为93.75%、81.25%、55.56%、98.11%、75.00%,均高于CA125+HE4、超声+ROMA。结论超声、CA125、HE4以及ROMA均不具备单独诊断良恶性卵巢癌的条件,而超声联合ROMA在诊断良恶性卵巢癌方面具有显著的优势,临床可以此为依据扩大样本量对其在临床分期中的诊断价值作进一步分析。
【Abstract】 Objective To explore the diagnostic value of ultrasonography combined with multiple blood indexes in ovarian malignant tumors.Methods From October 2015 to October 2018, 80 patients with ovarian tumors were diagnosed by pathological examination after surgery, and 40 healthy subjects who were examined by the physical examination center of the hospital in the same period. According to the diagnosis results,they were divided into benign tumor group and malignant tumor group.Then collected and analyzed the serum CA125, HE4, ROMA values and ultrasonic examination results of the two groups, and compared and analyzed the value of single index and combined detection in the diagnosis of early ovarian malignant tumor by statistical methods.Results The sensitivity of CA125 and ROMA was the highest in the diagnosis of benign and malignant tumors. The specificity of HE4 and ultrasound were respectively 96.88% and 90.63%. The positive predictive value of HE4 was up to 83.33%. All indexes of negative predictive value were above 91%, and the difference was not obvious.In terms of Youden index, there was no significant difference between CA125 and ultrasound, while HE4 and ROMA were relatively low. The sensitivity, specificity, positive predictive value, negative predictive value and Youden index of ultrasonography +ROMA in the diagnosis of benign and malignant ovarian cancer were 93.75%, 81.25%, 55.56%,98.11% and 75.00%, respectively. Conclusion The ultrasonography,CA125, HE4 and ROMA were not qualified to diagnose benign and malignant ovarian cancer alone, but ultrasonography combined with ROMA had significant advantages in the diagnosis of benign and malignant ovarian cancer, and its diagnostic value in clinical stages can be further analyzed based on the expanded sample size.
