注水法分离帽状腱膜在颅骨修补中的临床应用

【摘要】 目的观察注水法分离帽状腱膜与脑膜剪锐性分离帽状腱膜在颅骨修补术中的临床疗效差异。方法将2016年1月—2018年9月收治的48例颅骨缺损患者随机分成研究组与对照组,每组24例。研究组采用注水法分离帽状腱膜技术,对照组采用脑膜剪锐性分离帽状腱膜,观察两组病例手术时间、术中出血量、术后发热天数、术后引流量、术后是否发生脑脊液漏或皮下积液以及癫痫发作、术后切口感染等情况。结果研究组患者手术时间、术中出血量、术后引流量较对照组有所减少(P <0.05),但研究组患者术后发热情况、术后脑脊液漏或皮下积液、术后切口感染的发生率、术后癫痫发作率较对照组差异无统计学意义(P> 0.05)。结论注水法分离帽状腱膜技术可以减少颅骨修补术患者手术并发症,提高临床疗效。

【Abstract】 Objective To observe the difference of clinical efficacy of injection to separate the hat tendon membrane and meningeal scissor to separate the hat tendon membrane in cranioplasty. Methods 48 patients admitted to our hospital from January 2016 to September 2018 with skull defects were randomly divided into a study group and a control group, 24 cases in each group. The study group was applied injection method to separate the hat tendon membrane, the control group was separated with meningeal scissor, the time of operation, the amount of bleeding in operation, fever after operation, the drainage after operation, cerebrospinal fluid leakage or subcutaneous effusion occurred after operation, epilepsy, incision infection was observed. Results The time of operation, intraoperative hemorrhage and postoperative drainage was decreased in study group compared with control group(P < 0.05). But, there were no significant difference in postoperative fever, postoperative cerebrospinal fluid leakage or subcutaneous effusion, postoperative incision infection and postoperative epilepsy rate between two groups(P > 0.05). Conclusion The separation of the hat tendon membrane by injection technique can reduce the complications and improve the clinical effect of patients with cranioplasty.
