【摘要】 目的分析研究甲状腺激素水平的不同对产妇免疫功能指标及母儿妊娠结局的影响。方法于2016年8月至2019年11月挑选共600例实行产检产妇作为本次实验对象,按照产妇甲状腺激素水平(TSH)将其分为以下3组,分别包括:200例A组(甲亢)、200例B组(甲减)与200例参照组(甲状腺激素水平正常)。分别对以上3组产妇实行免疫功能检测与甲状腺激素检测,观察产妇免疫功能指标及母儿妊娠结局。结果 A组与B组和参照着产妇妊娠早、中、晚期的TSH水平显著偏低,而FT3与FT4水平相对比,A组产妇显著偏高(P<0.05);与B组和参照组产妇相比较,A组产妇妊娠早、中、晚期免疫功能指标显著偏高;与参照组妊娠结局相比较,A组与B组产妇足月分娩健康婴儿显著偏少,且剖宫产率偏高(P<0.05)。结论若产妇甲减或者是甲亢则会引起自身的免疫功能指标异常,严重影响母儿之间的妊娠结局,因此必须基于产妇早期甲状腺激素水平检测,并进行有效的控制。

【Abstract】 Objective To analyze the effects of different thyroid hormone levels on maternal immune function parameters and maternal and child pregnancy outcomes. Methods From August 2016 to November 2019, a total of 600 pregnant women undergoing maternity examination were selected as the subjects of this experiment. According to the maternal thyroid hormone level(TSH), they were divided into the following three groups, including: 200 cases of group A), 200 cases of group B(hypothyroidism) and 200 cases of reference group(positive thyroid hormone levels). The immune function and thyroid hormone of the three groups were detected respectively, and the immune function indexes and pregnancy outcome were observed. Results The TSH levels of group a, group b and the reference parturients in the early, middle and late trimester of pregnancy were significantly lower, while compared with FT3 and FT4 levels, group a parturients were significantly higher(P<0.05). Compared with group B and reference group, group A’s maternal immune function indexes were significantly higher in early, middle and late pregnancy. Compared with the pregnancy outcome of the reference group, the number of healthy full-term delivery infants in group a and group b was significantly lower, and the cesarean section rate was higher(P<0.05). Conclusion If maternal hypothyroidism or hyperthyroidism can cause abnormal immune function indexes and seriously affect the pregnancy outcome between mother and child, it must be based on early maternal thyroid hormone detection and effective control.
