全麻联合颈浅丛神经阻滞在甲状腺手术中的应用

【摘要】 目的探讨舒芬太尼复合罗哌卡因双侧颈浅丛神经阻滞对甲状腺手术术后镇痛及不良反应的影响。方法 120例择期行甲状腺手术患者,随机分为三组:A组全身麻醉组,B组罗哌卡因颈浅丛神经阻滞联合全麻组,C组舒芬太尼复合罗哌卡因颈浅丛神经阻滞联合全麻组,每组各40例。比较三组患者术中全麻药用量,术后镇痛效果及不良反应情况。结果三组术中全麻药用量差异有统计学意义(P<0.05),三组患者术后恶心呕吐不良反应差异有统计学意义(P=0.02)。结论舒芬太尼复合罗哌卡因颈浅丛神经阻滞联合全身麻醉不仅减少全麻药用量,而且术后恶心呕吐不良反应少,镇痛时间长。

【Abstract】 Objective The aim of this study is to evaluate the analgesic efficacy and possible side effects of bilateral superficial cervical plexus block(BSCPB) and the effect of sufentanil addition to ropivacaine in patients undergoing thyroid surgery. Methods 120 patients undergoing elective thyroid surgery were randomLy divided into three groups each of 40. Group A received general anesthesia, group B received cervical superficial plexus block with ropivacaine combined with general anesthesia and group C received sufentanil with ropivacaine cervical superficial plexus block combined with general anesthesia. The dosage of general anesthetic, postoperative analgesic effect and adverse reactions were compared among the three groups. Results Compared with group A, patients in group B and group C had lower consumption of anesthetics and lower side effect profile. The difference was statistically significant. Conclusion Bilateral superficial cervical plexus block(BSCPB) using a combination of sufentanil and ropivacaine was superior to the other two for prolonged analgesia with less intra and postoperative narcotic drugs consumption and lower side effect profile during thyroid surgery.
