喉癌切除术后咽瘘感染病原菌特点及耐药性分析
Pathogenic Bacteria Characteristics and Drug Resistance of Pharyngeal Fistula Infection after Laryngeal Cancer Resection
【摘要】 目的了解不同分型喉癌切除术后咽瘘感染情况,探讨感染病原菌特点及耐药性分析,为临床治疗提供参考依据。方法收集2011年1月至2018年12月在我院行喉癌切除手术的患者临床资料,分析不同临床分型喉癌术后咽瘘感染发生率,病原体来源及耐药性。结果共监测喉癌切除手术患者243例,发生感染39例,感染率为16.05%。声门上型喉癌术后咽瘘感染率最高,为30.51%;检出病原菌28株,以革兰阴性菌为主,占60.71%,其中革兰阳性球菌各菌属均对青霉素类、一代头孢菌素和庆大霉素、红霉素耐药率较高;革兰阴性杆菌各菌属均对青霉素类、头孢菌素类、β-内酰胺类、喹诺酮类抗菌药物耐药率较高。结论声门上型喉癌全喉切除术后咽瘘感染率较高,以革兰阴性菌为主,且耐药性较为严重。
【Abstract】 Objective To investigate the infection of pharyngeal fistula after different types of laryngeal cancer resection, and to explore the characteristics of pathogenic bacteria and drug resistance analysis, so as to provide reference for clinical treatment. Methods To collect the clinical data of patients who underwent laryngeal cancer resection in our hospital from January 2011 to December2018, and analyze the different clinical types of laryngeal cancer incidence, pathogen source and drug resistance of pharyngeal fistula infection. Results A total of 243 patients were monitoring laryngeal resection, 39 cases of infection, the infection rate was 16.05%, the treatment of supraglottic carcinoma pharyngeal fistula after the highest infection rate of 30.51%. The rate of pharyngeal fistula infection was the highest in supraglottic laryngeal cancer, which was 30.51%. 28 strains of pathogenic bacteria were detected, mainly was gramnegative bacteria, accounting for 60.17%. Gram-positive cocci were resistant to penicillins, first-generation cephalosporins, gentamicin and erythromycin. The resistance rates of gram-negative bacilli to penicillins, cephalosporins, beta-lactams and quinolones were higher.Conclusion The infection rate of pharyngeal fistula after total laryngectomy for supraglottic laryngeal cancer is higher, mainly Gramnegative bacteria, and the drug resistance is serious.
