不同储雾罐对硫酸沙丁胺醇定量吸入气雾剂体外沉积性质的影响

【摘要】 目的比较4款不同储雾罐对吸入气雾剂的体外沉积率和空气动力学粒径分布的影响。方法以硫酸沙丁胺醇吸入气雾剂(万托林)为模型药物,采用新一代药物撞击器(NGI)和呼吸模拟器对4种储雾罐进行体外沉积性质测试。结果和不加储雾罐的气雾剂气溶胶相比,加装储雾罐后的气溶胶质量平均空气动力学粒径(MMAD)和咽喉沉积率显著下降。不同储雾罐产生的MMAD存在显著差异,导致不同的微细粒子分数(FPF)和可吸入药物总量。其中成人吸入模式下,舒倍适储雾罐的递送总量(26.27±1.07)μg/揿显著优于其他三款储雾灌的16.97-21.36μg/揿。结论加装储雾罐可显著降低药物咽喉沉积量,但不同的储雾罐导致了不同递送总量和MMAD的气溶胶,以及可能会导致不同的临床疗效。

【Abstract】 Objective To compare the effects of four different spacers on in vitro deposition properties of Ventolin metered dose inhaler(MDI). Methods The aerodynamic properties and delivered dose were determined using the next generation pharmaceutical impactor and a breathing simulator, respectively. Results Relative to the MDI without a spacer, the presence of spacer significantly decreased the mass median aerodynamic diameters(MMAD) and the drug amount recovered in the induction port. However, the MMAD and delivered dose appeared to be affected spacer-dependent with the TEESPACER conferring significantly higher delivered dose than any other spacer at an adult breathing pattern. Conclusion The present results demonstrated that the presence of spacer significantly affected the in vitro deposition properties of MDI in a spacer-dependent manner, which may result in spacer-dependent clinical outcomes.
