关节置换患者术后便秘的相关因素分析
Analysis on related factors of postoperative constipation in patients with arthroplasty
【摘要】 目的探讨关节置换患者术后出现便秘的相关因素,为预防术后便秘提供依据。方法采用方便抽样法,选取2017年5月至2018年2月住院进行人工关节置换手术治疗的156例患者为研究对象,应用自设问卷对其进行调查,观察患者的便秘情况,统计分析可能导致关节置换患者术后发生便秘的相关因素。结果 156例关节置换患者中有121例发生了不同程度的便秘,发生率为77.56%(121/156)。单因素分析显示:年龄、文化程度、生活规律、饮食、心理状态——焦虑程度、疼痛、卧床时间、安置引流管、术前有无便秘及术后有无并发症发生对关节置换术后患者发生便秘有明显影响(P<0.05);多因素分析显示:高龄、生活不规律、饮食不规律、卧床时间增加、术前便秘、安置引流管增加了便秘风险,高教育水平降低了便秘风险。结论关节置换患者术后便秘的发生率较高,对于关节置换术患者,应在术前积极采取预防策略,降低关节置换术后便秘发生率,减轻患者痛苦,以促进疾病的早日康复。
【Abstract】 Objective: To explore the related factors of constipation after arthroplasty, and to provide evidence for preventing constipation after arthroplasty.Methods: 156 patients who underwent arthroplasty from May 2017 to February 2018 were selected by convenient sampling. A self-designed questionnaire was used to investigate the constipation of the patients. The related factors that might lead to constipation after arthroplasty were analyzed. Results: 121 out of 156 patients with joint replacement had different degrees of constipation, and the incidence was 77.56%(121/156). Univariate analysis showed that age, education level, life pattern, diet, psychological state-anxiety level, pain, bed rest time, placement of drainage tube, preoperative constipation and postoperative complications had a significant impact on constipation after joint replacement(P<0.05). Multivariate analysis showed that age, irregular life, irregular diet, increased bedtime, preoperative constipation, placement of drainage tube increased the risk of constipation, and high education level reduced the risk of constipation.Conclusion: The incidence of constipation after joint replacement is high. Preoperative preventive strategies should be taken to reduce the incidence of constipation after joint replacement and alleviate the pain of patients so as to promote the early recovery of the disease.
