冠心病合并2型糖尿病患者的临床特点分析

【摘要】 目的对冠心病合并2型糖尿病(T2DM)患者的临床特征表现进行系统探析。方法将2017年5月—2018年5月该院诊治的55例冠心病合并T2DM患者纳入研究组,选取同期诊治的55例单纯性冠心病患者纳入对照组,对比分析两组临床表现,并比较冠脉造影特征和心率变异(HRV)时域指标。结果研究组患者治疗后空腹血糖(FPG)、餐后2h血糖(2 hPG)均显著降低,与治疗前比较差异有统计学意义(P<0.05);研究组患者无症状性心肌缺血、高血压、高血脂的发生率均显著高于对照组(P<0.05);研究组病变冠脉以多支为主,且主要是弥漫性病变,与对照组比较,差异有统计学意义(P<0.05)。结论冠心病合并T2DM患者存在高血压、高血脂及心肌缺血等表现,且冠脉病变以多支、弥漫性病变为主,易引起心率变化。

【Abstract】 Objective The clinical characteristics of patients with coronary heart disease complicated with type 2 diabetes mellitus(T2DM) were systematically analyzed. Methods A total of 55 patients with coronary heart disease and T2DM treated in our hospital from May 2017 to May 2018 were enrolled in the study group. And 55 patients with simple coronary heart disease who were diagnosed and treated in the same period were enrolled in the control group. The clinical manifestations,pulse angiography and heart rate variability(HRV) time domain indicators of the two groups were compared and analyzed.Results The fasting blood glucose(FPG) and 2 h postprandial blood glucose(2 hPG) of the patients in the study group were significantly reduced after treatment, with statistically significant differences from those before treatment(P<0.05). The incidence of asymptomatic myocardial ischemia, hypertension and hyperlipidemia in the study group was significantly higher than that in the control group(P<0.05). The coronary artery lesions in the study group were mainly multivessel and mainly diffuse, with statistically significant differences compared with the control group(P<0.05). Conclusion Patients with coronary heart disease and T2DM have hypertension, hyperlipidemia and myocardial ischemia, and the coronary lesions are mainly multi-distributed and diffuse lesions, which may cause heart rate changes.
