糖尿病足周围血管神经病变的预防和治疗研究
Prevention and Treatment of Vascular Neuropathy Around Diabetic Foot
【摘要】 目的探究糖尿病足周围血管神经病变的预防与治疗措施。方法选取2016年5月—2019年1月期间到笔者所在医院接受治疗的52例糖尿病足周围血管神经病变患者为该次研究对象,随机分为对照组(n=26例,常规治疗)、研究组(n=26例,甲钴胺+依帕司他),比较两组治疗后糖代谢水平、神经传导速度。结果治疗后研究组患者的HbA1c为(5.33±1.31)、P2HBG为(8.17±2.55)、FBG为(5.27±1.11)各项血糖指标优于对照组患者的HbA1c (8.31±1.33)、P2HBG(10.41±2.67)、FBG(7.61±2.17)差异有统计学意义(P<0.05);治疗后研究组患者的正中神经的传导速度为(49.13±8.97),腓神经的传导速度为(44.67±9.17),比对照组患者的正中神经的传导速度(44.03±7.61),腓神经的传导速度(39.43±8.05),差异有统计学意义(P<0.05)。结论在对糖尿病足周围血管神经病变治疗中,采用甲钴胺+依帕司他治疗可帮助患者有效的改善糖代谢水平、加快神经传导速度,值得临床上推广及应用。
【Abstract】 Objective To explore the prevention and treatment of vascular neuropathy around diabetic foot. Methods A total of 52 patients with diabetic peripheral vascular neuropathy who were treated in the hospital from May 2016 to January 2019 were randomly divided into the control group(n=26 cases, routine treatment). The study group(n=26 cases,mecobalamin + epalrestat), compared the glucose metabolism level and nerve conduction velocity after treatment. Results After treatment, the HbA1 c of the study group was(5.33±1.31), P2 HBG was(8.17±2.55), and FBG was(5.27±1.11). The blood glucose index was better than that of the control group. HbA1 c(8.31 ±1.33), P2 HBG(10.41±2.67), FBG(7.61±2.17)The difference was statistically significant(P <0.05). After treatment, the conduction velocity of the median nerve was(49.13±8.97), and the conduction velocity of the phrenic nerve was(44.67±9.17). The conduction velocity of the median nerve was(44.03±7.61) compared with the control group, the conduction velocity of the phrenic nerve was(39.43±8.05),and the difference was significant, P <0.05. Conclusion In the treatment of diabetic peripheral vascular neuropathy,treatment with mecobalamin + epalrestat can help patients effectively improve the level of glucose metabolism. Accelerate the nerve conduction velocity, which is worthy of clinical promotion and application.
