【摘要】 目的探讨腹腔镜手术和开腹手术在胆囊炎合并下肢深静脉血栓患者中的疗效对比。方法选择2016年6月—2018年11月治疗的胆囊炎合并下肢深静脉血栓患者76例作为对象,随机数字法分为对照组(n=38例)和观察组(n=38例)。对照组给予开腹手术治疗,观察组采用腹腔镜手术治疗,术后3 d对患者效果进行评估,比较两组手术指标、炎症因子水平。结果观察组手术时间长于对照组,差异有统计学意义(P<0.05);观察组住院时间、切口长度、术中出血量、下床活动时间,短(少)于对照组,差异有统计学意义(P<0.05);观察组手术后3 d炎症因子TNF-a、IL-2、CRP水平低于对照组,差异有统计学意义(P<0.05)。结论腹腔镜手术用于胆囊炎合并下肢深静脉血栓患者中手术创伤较小,能降低炎症因子水平,值得推广应用。更多还原

【Abstract】 Objective To compare the efficacy of laparoscopic and open surgery in patients with cholecystitis and deep venous thrombosis of lower extremities. Methods 76 patients with cholecystitis complicated with deep venous thrombosis of lower extremities treated from June 2016 to November 2018 were randomly divided into control group(n=38 cases) and observation group(n=38 cases). The control group was treated with open surgery, while the observation group was treated with laparoscopic surgery. The effect of the patients was evaluated 3 days after the operation, and the levels of surgical indicators and inflammatory factors were compared between the two groups. Results The operation time of the observation group was longer than that of the control group,the difference was statistically significant(P<0.05); the hospitalization time, incision length, intraoperative bleeding volume and time of getting out of bed in the observation group were shorter than those in the control group,the difference was statistically significant(P<0.05); the levels of inflammatory factors TNF-a, IL-2 and CRP in the observation group were lower than those in the control group,the difference was statistically significant(P<0.05). Conclusion Laparoscopic cholecystitis combined with deep venous thrombosis of lower extremities has less trauma and can reduce the level of inflammatory factors, which is worthy of popularization and application.更多还原
