【摘要】 目的探讨球囊扩张辅助导管溶栓治疗65岁以上患者急性髂股静脉血栓的效果及可行性。方法回顾分析2012年1月至2017年3月期间我科收治的78例65岁以上急性髂股静脉血栓患者经3种不同治疗方案分为3组,A组:系统溶栓组,B组:导管溶栓组,C组:球囊辅助导管溶栓组;分析3组患者相关数据。结果所有患者症状均得到不同程度缓解,溶栓时间、尿激酶总量、并发症发生率B、C组优于A组(P <0.05),C组优于B组(P <0.05);治疗前后下肢周径差变化、静脉通畅度评分、静脉通畅率及静脉血栓后遗症发生率B、C组明显优于A组(P <0.05),B、C两组间差异无统计学意义(P> 0.05)。结论导管溶栓对治疗65岁以上患者髂股静脉血栓效果明确、安全、经济,可降低深静脉血栓后遗症形成的概率,预先球囊扩张可以缩短溶栓治疗时间,减少溶栓药物剂量及并发症的发生,值得推广。更多还原

【Abstract】 Objective To evaluate the efficacy and safety of balloon-assisted catheter-directed thrombolysis(CDT)treat for acute iliofemoral venous thrombosis(IFVT)of patients age more than 65 years. Methods 78 patients with IFVT treated at People′s Hospital of Jiang xi Province from January 2012 to March 2017 were retrospectively analyzed. The patients were divided into three groups(Group A:system thrombolysis;Group B:treat by CDT;Group C:treat by balloon-assisted CDT). Results All patients with limb swelling achieved varying degrees of regression. Regarding the thrombolysis time,the total dose of Urokinase,the complication,group B and C were less than group A,and group C was less than B(P < 0.05). Regarding preoperative and postoperative thigh/calf circumference,venous patency score,the rate of venous patency,the post-thrombotic syndrome(PTS)in patients,group B and C were better than A,but B was no different from C. Conclusions Balloon-assisted CDT treat for IFVT in the patients over 65 years old is reliable,safe and minimally invasive surgery,which can cut down thrombolysis time and reduce the total dose of urokinase and the complications.更多还原
