拓扑异构酶Ⅱα与卵巢浆液性癌的相关性研究
Study on the correlation between TOP2A and ovarian serous carcinoma
【摘要】 目的探讨拓扑异构酶Ⅱα(TOP2A)与卵巢浆液性癌(OSC)的相关性,并分析其临床病理学意义。方法回顾性分析2000年1月至2017年1月期间北京市平谷区医院61例OSC手术标本及相应临床病理资料,通过免疫组织化学技术检测TOP2A蛋白表达情况,运用荧光原位杂交(FISH)技术检测TOP2A基因拷贝数的变化,并采用Spearman等级相关性分析检测结果与其临床病理资料的相关性。结果在61例OSC标本中,组织病理学诊断为低级别20例(32. 8%),高级别41例(67. 2%); TOP2A蛋白表达阳性者48例(78. 7%),其中高表达者31例(50. 8%)。在高级别OSC和临床分期较晚的标本中,TOP2A蛋白表达量显著增加(P值分别为0. 006、0. 001)。FISH检测结果显示,36例(59. 0%)存在TOP2A基因的扩增,统计学分析显示,TOP2A基因更易在高级别OSC(χ~2=10. 358,P <0. 05)、临床III期及IV期(χ~2=10. 134,P <0. 05)病例中发生扩增。此外,TOP2A蛋白阳性表达与TOP2A基因扩增呈正相关(r=0. 412,P=0. 006)。结论 OSC标本的TOP2A蛋白阳性表达与TOP2A基因扩增呈正相关,并且可能与OSC的恶性程度和肿瘤发生及发展相关。
【Abstract】 Objective To explore the correlation between TOP2 A and ovarian serous carcinoma,and to analyze their clinical and pathological significance. Methods Retrospective analysis of 61 OSC surgical specimens and corresponding clinicopathological data of Pinggu District Hospital in Beijing during January 2000 to January 2017 were carried out. Immunohistochemical staining was used to detect the expression level of TOP2 A protein and fluorescence in situ hybridization( FISH) was used to detect the genetic changes of TOP2 A. The correlation between results of statistical analysis and corresponding clinic pathological data was analyzed by Spearman level correlation analysis. Results In 61 cases of ovarian serous carcinoma,which was diagnosed as low grade iin 20 cases( 32. 8%) and high grade in 41 cases( 67. 2%),and their TOP2 A protein was positively expressed in 48 cases( 78. 7%) and highly expressed in 31 cases( 50. 8%). The expression of TOP2 A protein was significantly increased in high grade OSC( P = 0. 006) and expression in specimens of late clinical stage( P = 0. 001). The results of FISH test showed that amplification of TOP2 A gene was present in 36 cases( 59. 0%). The statistical analysis showed that TOP2 A gene was more likely to be amplified in high level of OSC( χ~2= 10. 358,P < 0. 05),clinical stages in III and IV( χ~2= 10. 134,P < 0. 05). In addition,the expression of TOP2 A protein was positively correlated with the amplication of TOP2 A gene( r = 0. 412,P = 0. 006). Conclusion The positive expression of TOP2 A protein and amplification of TOP2 A gene in OSC patients may be related to the malignancy of OSC and the occurrence and development of tumor.
