肿块型乳腺癌的MRI征象与淋巴结转移相关性研究
Correlation between MRI characteristics and lymph node metastasis of the mass-type breast cancer
【摘要】 目的 探讨肿块型乳腺癌的MRI征象与腋窝淋巴结转移的关系。方法 回顾性分析187例肿块型乳腺癌的MRI征象,术后腋窝淋巴结转移的情况,根据淋巴结转移个数多少分为pN0、pN1、pN2、pN3 4组。分析每组乳腺癌MRI征象与N病理分期的相关性。结果 pN0组108例,pN1组33例,pN2组22例,pN3组24例。在4组中,类圆形肿块分别为15、5、1、1例;分叶状肿块分别为8、1、1、0例;不规则肿块分别为85、27、20、23例,各组间肿块形态无显著性差异(P>0.05)。肿块边界清楚分别为7、0、1、0例;边缘不规则分别为69、14、7、8例;边缘毛刺分别为32、19、14、16例,各组间肿块边缘有显著性差异(P<0.05)。肿块不均匀强化分别为55、16、14、18例;环形强化分别为43、14、5、6例;中心强化分别为3、1、1、0例;分隔状强化分别为7、2、2、0例,各组间强化方式无显著性差异(P>0.05)。Ⅰ、Ⅱ、Ⅲ型TIC和ADC值在不同组间均无显著性差异(P>0.05)。结论 肿块型乳腺癌MRI影像特征判断腋窝淋巴结转移价值有限,肿块边界为毛刺判断腋窝淋巴结转移有一定价值。
【Abstract】 Objective To explore the correlation between MRI characteristics and axillary lymph node metastasis of mass-type breast cancer.Methods MRI characteristics and pathological results of postoperative axillary lymph node metastasis in 187 cases with mass-type breast cancer were analyzed retrospectively.According to the number of lymph node metastases, all of the patients were divided into the four groups: pN0,pN1,pN2 and pN3.The correlation between MRI signs and N pathological stages in each group were analyzed.Results There were 108 cases in pN0 group, 33 cases in pN1 group, 22 cases in pN2 group and 24 cases in pN3 group.In the different groups, there were 15,5,1 and 1 case with round mass respectively; 8,1,1 and 0 case with lobular masse respectively; 85,27,20 and 23 cases with irregular masse respectively.The shapes were not significantly different among the different groups(P>0.05).In the different groups, there were 7,0,1 and 0 case with clear margin respectively; 69,14,7 and 8 cases with irregular margin respectively; 32,19,14 and 16 cases with spiculated margin respectively.The margins were significantly different among the different groups(P<0.05).In the different groups, there were 55,16,14 and 18 cases with heterogenous enhancement respectively; 43,14,5 and 6 cases with ring enhancement respectively; 3,1,1 and 0 case with central enhancement respectively; 7,2,2 and 0 case with septum enhancement.The enhancement patterns were not significantly different among the different groups(P>0.05).The numbers of the mass with type Ⅰ,Ⅱ and Ⅲ time-signal curves and the ADC values were not significantly different among the different groups(P>0.05).Conclusion MRI features of mass-type breast cancers have limited value in determining axillary lymph node metastasis.However, the spiculated margin of mass is valuable in determining axillary lymph node metastasis.
