MR IDEAL技术在椎体压缩性骨折病因分析中的价值

【摘要】 目的 探讨MR非对称回波三点法水脂分离技术(IDEAL)在良恶性椎体压缩性骨折中的鉴别价值。方法 利用GE 1.5T超导MR扫描仪(GE Brivo 355 MR System)对48例椎体压缩性骨折患者的85个椎体进行常规MR扫描和IDEAL扫描,其中良性骨折25例,42个椎体(包括单纯外伤8例,骨质疏松16例,结核1例),恶性骨折23例,43个椎体(包括转移性肿瘤22例,骨髓瘤1例)。测量并计算2组椎体在水脂像、同反相位的信号强度比(脂像信号强度/水像信号强度,反相位信号强度/同相位信号强度),即信号比值(SIR),并对4组SIR进行统计学分析。结果 椎体良性压缩骨折水脂像SIR 0.67±0.19,恶性压缩骨折水脂像SIR 0.14±0.07,两者差异有统计学意义(t=12.62,P<0.05);椎体良性压缩骨折同反相位SIR 0.52±0.25,恶性压缩骨折同反相位SIR 0.83±0.07,两者差异有统计学意义(t=-7.29,P<0.05)。结论 恶性组水脂像SIR明显低于良性组,恶性组同反相位像SIR高于良性组,良恶性水脂像、同反相位像SIR无重叠。IDEAL对良恶性椎体压缩骨折的鉴别有较高的应用价值,值得推广使用。

【Abstract】 Objective To discuss the differential value of magnetic resonance asymmetric echo three-point water-fat separation technique(IDEAL) in benign and malignant vertebral compression fractures.Methods 48 cases of vertebral compression fractures patients(85 vertebrals) were performed conventional MRI and IDEAL scanning at 1.5T superconducting MR scanner(GE Brivo 355MR system).25 benign and 23 malignant vertebral compression fracture cases were enrolled in this study.25 benign cases(42 vertebrals)included8 pure traumatic fractures,16 osteoporosis and 1 tuberculosis respectively,while 23 malignant cases(43 vertebrals) included 22 metastases and 1myeloma respectively.The signal intensity ratios(SIR) of the benign and malignant vertebral in water-fat imaging and that of in-phase and out-phase(fatty signal intensity/water signal intensity and out-phase signal intensity/in-phase signal intensity respectively),and namely SIR were measured and calculated.SIR in the 4 groups were statistically analyzed.Results Water-fat SIR of benign vertebral compression fracture was 0.67±0.19,whereas in malignant vertebral compression fractures was 0.14±0.07,and the difference was statistically significant(t=12.62,P<0.05).SIR of out-phase signal intensity/in-phase signal intensity of benign vertebral compression fracture was 0.52±0.25,whereas in malignant vertebral compression fracture was 0.83±0.07,and the difference was statistically significant(t=-7.29,P<0.05).Conclusion The SIR of malignant group is significantly lower than that of benign group in water-fat imaging.whereas the SIR of malignant group is higher than that of benign group in in-phase and out-phase imaging.There is no overlap of SIR between benign and malignant in water-fat imaging and in-phase and out-phase imaging.IDEAL technique has a high application value in the identification of benign and malignant vertebral compression fractures,which is worth popularizing.
