【摘要】 目的 探讨MRI信号强度与垂体腺瘤（PA）质地的相关性及其对质地的预测价值。方法 47例经手术病理证实的PA患者根据质地分为2组：质软组37例，质韧组10例。比较2组患者的T₁WI、T₂WI相对信号强度及动态增强参数[信号增强率（SER）],分析信号强度和动态增强参数与质地之间的关系。结果 2组间PA的T₂信号强度[T₂PA/T₂脑白质（WM）、T₂PA/T₂脑灰质（GM）、T₂PA/T₂脑脊液（CSF）]及SER有显著的统计学差异（P<0.05）:质软组显著高于质韧组；2组间T₁信号强度[T₁PA/T₁脑桥（PN）]无显著统计学差异（P>0.05）。绘制T₂PA/T₂WM、T₂PA/T₂GM、T₂PA/T₂CSF、T₁PA/T₁PN及SER对PA质地的预测ROC曲线，结果显示：AUC分别为0.857、0.835、0.856、0.630、0.781,其中T₂PA/T₂WM的AUC最大（P<0.05）。结论 MRI T₂信号强度与PA质地有关，T₂PA/T₂WM、T₂PA/T₂GM、T₂PA/T₂CSF可作为判断PA质地的指标。更多还原

【Abstract】 Objective To evaluate the predictive value of preoperative MRI signal intensity in different textures of pituitary adenoma（PA）.Methods According to the inclusion criteria,47 cases of PA were collected,including 37 soft cases and 10 firm cases.Different textures of PA on MRI signal intensity and dynamic enhancement characteristics were analyzed.Results T₂ WI intensity between two texture groups（T₂ PA/T₂ WM、T₂ PA/T₂ GM、T₂ PA/T₂ CSF and SER） had significant statistical differences（P <0.05）.T₁ WI intensity between two texture groups（T₁ WIPA/T₁ WIPN） had no significant statistical difference（P>0.05）.The T₂ signal intensity（T₂ PA/T₂ WM、T₂ PA/T₂ GM、T₂ PA/T₂ CSF） and SER in soft group was higher than that in the firm group.Predictive value of the texture of PA by using the ROC curve of T₂ PA/T₂ WM,T₂ PA/T₂ GM,T₂ PA/T₂ CSF,T₁ PA/T₁ PN and SER were obtained,and the areas under the ROC curve were 0.857,0.835,0.856,0.630 and 0.781 respectively.The largest area under the ROC curve was T₂ PA/T₂ WM（P<0.05）.Conclusion MRI T₂ signal intensity is related to the texture of PA and those indexes can be used to judge the texture of PA.更多还原
