甲状腺肿瘤微血管密度与超声表现的相关性

【摘要】 目的探讨甲状腺肿瘤微血管密度与超声表现的相关性。方法选取的98例甲状腺肿瘤患者均采用多普勒超声仪检查及免疫组化染色,计算甲状腺肿瘤微血管密度,分析甲状腺肿瘤微血管密度与超声表现的相关性。结果通过超声造影检查,得到56例甲状腺癌中55例超声表现不均匀低增强,1例表现稍高增强。22例甲状腺肿中16例表现与周围甲状腺实质同步等增强,4例表现出高增强,2例为低增强。20例甲状腺瘤中18例表现出高增强,2例表现为等增强。甲状腺癌的IMAX与甲状腺肿、甲状腺瘤相比,差异有统计学意义(P <0. 05)。IMAX与MVD两者呈现正相关(P <0. 05)。结论甲状腺癌的增强模式为低增强,且呈乏供血状态,通过超声造影与免疫组化染色可为甲状腺肿瘤的诊断提供参考依据。

【Abstract】 Objective To investigate the correlation between microvessel density and ultrasonographic findings in thyroid tumors. Methods The clinical data of 98 patients with thyroid tumor treated were analyzed. All the patients were treated with Doppler ultrasonography and immunohistochemical staining to calculate the microvascular density of thyroid tumor,and to analyze the correlation between the microvascular density of thyroid tumor and the ultrasound performance. Results Through ultrasound contrast examination,55 cases of thyroid carcinoma showed abnormal and low enhancement in 56 cases,and 1 cases showed slightly higher enhancement. In 22 cases of goiter,16 cases showed synchronous enhancement with the surrounding thyroid parenchyma,4 cases showed high enhancement,and 2 cases showed low enhancement. In 20 cases of thyroid adenoma,18 cases showed high enhancement,2 cases showed equal enhancement. The IMAX of thyroid carcinoma was significantly different from that of goiter and thyroid tumor( P < 0. 05). There is a positive correlation between IMAX and MVD( P < 0. 05). Conclusion The enhanced mode of thyroid carcinoma is low enhancement and has a state of lack of blood supply. The diagnosis of thyroid tumor can be provided by ultrasound contrast and immunohistochemical staining.
