【摘要】 目的分析肺炎克雷伯菌(Klebsiella pneumoniae)感染所致肝脓肿侵袭综合征的病例资料并复习相关文献,以提高对此疾病的认识。方法回顾性分析10例确诊为肺炎克雷伯菌感染所致肝脓肿侵袭综合征患者的临床特点、治疗反应及转归,复习相关文献并探讨发病机制。结果 10例患者均有肝外播散性病灶,符合肝脓肿侵袭综合征的临床特征。10例患者均有肝脓肿,均合并眼内炎并造成视力障碍甚至失明,有3例同时并发肺脓肿,其中1例合并心包、胸腔积液及感染性心内膜炎;8例有糖尿病病史,2例无基础疾病。结论肺炎克雷伯菌感染所致的肝脓肿易引起肝外播散性病灶,好发于糖尿病及免疫缺陷人群,也可发生于免疫正常人群,其病情进展迅速,临床危害严重,早期诊断及治疗至关重要。更多还原

【Abstract】 Objective To analyze the clinical data of liver abscess invasion syndrome caused by Klebsiella pneumoniae infection and to review relevant literature so as to improve the understanding of this disease. Methods The clinical characteristics,treatment and prognosis of 10 patients with liver abscess invasion syndrome infection caused by Klebsiella pneumoniae infection were analyzed retrospectively.The related literature was reviewed to discuss the pathogenesis. Results All the 10 patients had extrahepatic disseminated lesions,which accords with the clinical features of liver abscess invasion syndrome.All the 10 patients had liver abscess,which combined with endophthalmitis and caused visual impairment or blindness.These cases were concurrent with pulmonary abscess,of which1 cases were complicated with pericardium,pleural effusion and infective endocarditis;8 cases had diabetes history and 2 cases had no basic disease. Conclusions The liver abscess caused by Klebsiella pneumoniae infection is easy to cause an extrahepatic spread focus,which can be found in people with diabetes and immunodeficiency,and can also occur in the normal immune population.The disease is progressing rapidly and the clinical harm is serious.Early diagnosis and treatment are very important.更多还原
