合并左心室肥厚对高血压患者血清miRNA-455水平的影响及意义
Effect and significance of serum miRNA-455 in hypertensive patients with left ventricular hypertrophy
【摘要】 目的观察合并左心室肥厚对高血压患者血清微小RNA(miRNA)-455水平的影响,分析miRNA-455水平与心肌纤维化指标的相关性。方法选取合并左心室肥厚的高血压患者60例为病例组、单纯高血压患者80例为病例对照组,另选50例健康体检者为健康对照组。采用实时荧光定量PCR法检测三组血清miRNA-455;采用ELISA法检测心肌纤维化指标,包括基质金属蛋白酶9(MMP-9)、基质金属蛋白酶组织抑制因子1(TIMP-1)、Ⅰ型胶原氨基端肽(PⅠNP)、Ⅲ型胶原氨基端肽(PⅢNP)。并对miRNA-455水平与心肌纤维化指标进行相关性分析。结果病例组和病例对照组血清miRNA-455相对表达量低于健康对照组,且病例组低于病例对照组(P均<0.05)。病例组、病例对照组血清PⅠNP、PⅢNP、MMP-9、TIMP-1水平高于健康对照组,且病例组PⅠNP、PⅢNP、MMP-9、TIMP-1水平高于病例对照组(P均<0.05)。病例组、病例对照组MMP-9/TIMP-1低于健康对照组,且病例组MMP-9/TIMP-1低于病例对照组(P均<0.05)。对所有研究对象的血清miRNA-455表达水平与血清纤维化指标进行相关性分析发现,血清miRNA-455表达水平与血清PⅠNP、PⅢNP、MMP-9、TIMP-1水平均呈负相关(r分别为-0.759、-0.823、-0.630、-0.682,P均<0.05)。结论合并左心室肥厚的高血压患者血清miRNA-455表达水平均低于正常,且低于单纯高血压患者;miRNA-455表达与心肌纤维化指标水平呈负相关。
【Abstract】 Objective To observe the effect of left ventricular hypertrophy on serum miRNA-455 level in hypertensive patients, and to analyze the correlation between miRNA-455 level and myocardial fibrosis indicators.Methods Sixty hypertension patients with left ventricular hypertrophy(case group), 80 simple hypertension patients(case control group) and 50 healthy subjects(healthy control group) were selected. The serum miRNA-455 was detected by real-time fluorescent quantitative PCR. The indexes of myocardial fibrosis were determined by ELISA, including serum metalloproteinase-9(MMP-9), matrix metalloproteinase-1(TIMP-1), propeptide of type I procollagen(PⅠNP), propeptide of type III procollagen(PⅢNP). The correlation between miRNA-455 level and fibrosis was analyzed. Results Compared with the healthy control group, the serum miRNA-455 levels in both the case group and the case control group decreased significantly, with the most significant decrease in the case group, and the difference was statistically significant(all P<0.05). Compared with the healthy control group, the serum levels of PⅠNP, PⅢNP, TIMP-1, and MMP-9 increased significantly in both the case group and the case control group, with the most significant increase in the case group, and the difference was statistically significant(all P<0.05). Compared with the healthy control group, the serum levels of MMP-9/TIMP-1 significantly decreased in both the case group and the case control group, with the most significant decrease in the case group, and the difference was statistically significant(all P<0.05). The expression level of miRNA-455 was negatively correlated with the serum levels of PⅠNP, PⅢNP, MMP-9 and TIMP-1(r=-0.759,-0.823,-0.630, and-0.682, respectively; all P<0.05). Conclusion Serum miRNA-455 expression level in the hypertensive patients with left ventricular hypertrophy is lower than the normal one and is even lower than that in patients with simple hypertension; there is a negative correlation between miRNA-455 expression and myocardial fibrosis indexes.
