外源性成纤维细胞生长因子联合引导骨再生术在前牙种植修复中对种植区骨量、种植体周围炎的影响

【摘要】 目的探讨外源性成纤维细胞生长因子(FGF)联合引导骨再生术(GBR)在前牙种植修复中的应用价值。方法选取我院拟实施前牙种植修复的患者40例,收集时间2015年1月至2016年8月,根据治疗方法分为FGF组、对照组各20例,FGF组采用FGF+GBR+牙种植术,对照组采用GBR+牙种植术,对比两组治疗前后牙槽骨唇舌(腭)向宽度、菌斑指数(PLI)、牙龈出血指数(SBI)、牙龈探诊深度(PD)。结果术前FGF组和对照组患者的牙槽骨唇舌(腭)向宽度测定值差异无统计学意义(P>0. 05);术后3个月、术后6个月,两组患者的术后牙槽骨唇舌(腭)向宽度测定值较术前均提高,FGF组患者的术后牙槽骨唇舌(腭)向宽度测定值均大于同一时间点的对照组,差异有统计学意义(P<0. 05);术前,FGF组和对照组的SBI、PD、PLI测定值差异无统计学意义(P>0. 05);术后6个月,FGF组患者的SBI、PD、PLI测定值均低于的对照组,两组患者的PD、PLI测定值较本组术前均呈一定程度的提高,对照组患者的SBI较本组术前显著提高,差异有统计学意义(P<0. 05)。结论 FGF联合GBR在列牙缺损种植体治疗中,有利于促进骨量增加,同时可减轻术后患者牙龈炎症反应。

【Abstract】 Objective To explore the application value of exogenous fibroblast growth factor( FGF) combined with guided bone regeneration( GBR) in anterior dental implant restoration.Methods We enrolled 40 cases of anterior dental implant restoration in our hospital from January 2015 to August 2016. According to the treatment method,we divided them into FGF group and control group,20 cases in each group.FGF group was treated with fibroblast+GBR+dental implant,while control group was treated with GBR+dental implant,then we compared the alveolar bone,lip and tongue( palate) width,plaque index( PLI),sulcus bleeding index( SBI) and gingival probing depth( PD) before and after treatment. Results There was no significant difference in alveolar bone lip-tongue( palate) width between the two groups before treatment( P>0. 05).The alveolar bone lip-tongue( palate) width of the two groups increased after 3 months and 6 months after treatment,and the alveolar bone,lip and tongue( palate) orientation width in the FGF group were higher than those in the control group at the same time point( P<0. 05).There was no significant difference in SBI,PD and PLI between the FGF group and the control group before treatment( P>0. 05).Six months after treatment,the SBI,PD and PLI values of the patients in the fibroblast growth factor group were lower than those of the control group.The values of PD and PLI of the patients in the two groups were increased to a certain extent compared with those before the treatment,and the SBI in the control group was significantly higher than that before the treatment,and the difference was statistically significant( P<0. 05).Conclusion FGF combined with GBR in the treatment of dental implant restoration can promote bone mass increase,and reduce gingival inflammation after treatment.
